A clear, colorless or slightly yellowish liquid with a slight lavender odor. The main active ingredient 1 ml of the drug contains phenylephrine 2.5 mg, dimethindene maleate 0.25 mg Product form Nasal spray Dosage 10 ml Pharmacological properties Pharmacodynamics Milt is a combined preparation containing phenylephrine and dimethindene. The drug reduces nasal discharge and helps to cleanse the nasal passages without disturbing the physiological functions of the ciliated epithelium and nasal mucosa. Phenylephrine belongs to the sympathomimetic amines. It is used as a nasal decongestant with a moderate vasoconstrictive effect, selectively stimulates α1-adrenergic receptors of the cavernous venous tissue of the nasal mucosa. Thus, it quickly and permanently eliminates swelling of the nasal mucosa and paranasal sinuses. Dimetindene is a histamine H1 receptor antagonist that exhibits anti-allergic effects. Effective at low doses, well tolerated. PharmacokineticsMilt is applied topically, so its activity does not correlate with the concentration of active substances in the blood plasma. With accidental oral administration, the bioavailability of phenylephrine decreased and amounted to approximately 38%, the elimination half-life was about 2.5 hours. The systemic bioavailability of dimethindene after oral administration in solution is about 70%, the half-life is about 6 hours. Indications for use Symptomatic treatment of colds, nasal congestion, acute and chronic rhinitis, seasonal (hay fever) and non-seasonal allergic rhinitis, acute and chronic sinusitis, vasomotor rhinitis. Auxiliary therapy for acute otitis media. Preparation for surgery in the nasal area and elimination of swelling of the nasal mucosa and paranasal sinuses after surgery. Route of administration and doses Before the introduction of the drug, you should thoroughly clean your nose. Adults and children over 6 years: 1 injection in each nasal passage 3-4 times a day. The bottle should be held vertically, with the spray up. Holding the head straight, insert the tip into the nasal passage, press the sprayer with a short sharp movement and, pulling it out of the nose, unclench it. During injection, it is recommended to inhale slightly through the nose. If there is no improvement after 3 days of use or symptoms worsen, you should consult a doctor. The duration of treatment should not exceed 7 days and depends on the course of the disease. Use during pregnancy and lactation The drug is not prescribed during pregnancy. Breastfeeding women are not prescribed the drug. Precautions Milt should not be used for longer than 1 week. If there is no improvement after 3 days of use or symptoms worsen, you should consult a doctor. In patients with hypersensitivity to sympathomimetics, insomnia, dizziness, tremors, cardiac arrhythmias, arterial hypertension may occur. With prolonged or excessive use, tachyphylaxis, rhinitis by the rebound mechanism (drug rhinitis) may develop. As with other vasoconstrictors, the recommended dose should not be exceeded. Excessive use of the drug, especially in children and the elderly, may cause systemic manifestations of the drug. Care should be taken to prescribe the drug to patients with cardiovascular diseases, arterial hypertension, angle-closure glaucoma, hyperthyroidism, diabetes mellitus, obstruction of the bladder neck (for example, with prostate enlargement). Patients with epilepsy should be taken with caution because the drug contains dimethindene maleate, a histamine H1 receptor antagonist. Interaction with other drugs The drug is contraindicated in patients who are taking monoamine oxidase inhibitors or have taken them in the previous 14 days. Vasoconstrictors should be used with caution in patients taking tricyclic antidepressants and antihypertensive drugs such as β-blockers. Their simultaneous administration may enhance the hypotensive effect of phenylephrine. Contraindications Hypersensitivity to any of the components of the drug. Due to the content of phenylephrine, this drug, like other vasoconstrictors, is contraindicated in atrophic rhinitis, in patients taking monoamine oxidase inhibitors or taking them in the previous 14 days, and in patients with angle-closure glaucoma. Ingredients: 1 ml of the drug contains phenylephrine 2.5 mg, dimethindene maleate 0.25 mg; excipients: benzalkonium chloride; lavender distillate; sorbitol (E420); citric acid monohydrate; disodium phosphate dodecahydrate; water for injections. Overdose In case of overdose, anticholinergic and sympathomimetic effects may increase: palpitations, ventricular extrasystoles, sinus tachycardia, arterial hypertension, headache, tremor, dilated pupils, dizziness, pallor, agitation, convulsions, hallucinations, insomnia; sedation, fatigue, abdominal pain, coma, nausea, and vomiting may also occur. There were no serious symptoms of overdose due to ingestion. Accidental ingestion of dimethindene up to 20 mg did not cause the development of serious overdose symptoms. In case of accidental use of the drug by young children, no serious side effects have been reported. In children, taking the whole vial (15mg) or tube (12mg) has not caused serious symptoms. Treatment: use of activated charcoal; possible use of laxatives for young children (gastric lavage is not required); adults and older children are prescribed the use of large amounts of fluid.
