The main active substanceOxymetazoline Release formNasal spray Dosage 0.05% 15 ml Pharmacological propertiesOxymetazoline hydrochloride is a synthetic adrenomimetic. The vasoconstrictor effect is manifested by a decrease in blood flow, a decrease in swelling of the mucous membranes of the nasal passages, paranasal sinuses and the Eustachian tube. This restores breathing through the nose, disturbed by the flu, colds and allergic diseases. Local narrowing of the vessels of the mucous membranes of the nose and paranasal sinuses occurs after 5? 10 minutes after injection of the drug into the nasal cavity. Indications for use Acute, allergic and vasomotor rhinitis; to restore drainage in case of inflammation of the paranasal sinuses of the nasal cavity, eustachitis, otitis media; to eliminate edema before diagnostic manipulations in the nasal passages. Benzalkonium chloride and propylene glycol, which are part of the excipients, can cause allergic skin reactions (irritation, itching). Benzalkonium chloride with prolonged use can cause swelling of the nasal mucosa. Interaction with other drugs The combined use of oxymetazoline and tricyclic antidepressants, maprotiline, MAO inhibitors, pressor agents can lead to an increase in blood pressure. Therefore, the combined use of these agents should be avoided. ContraindicationsIncreased individual sensitivity to the ingredients of the drug or other adrenergic drugs; atrophic rhinitis; inflammation of the skin and mucous membrane of the vestibule of the nose and the formation of crusts (dry rhinitis); arterial hypertension in severe form; severe atherosclerosis; tachycardia; cardiac asthma; increased intraocular pressure, angle-closure glaucoma; hyperthyroidism; diabetes; pheochromocytoma; severe renal dysfunction; difficulty urinating associated with an enlarged prostate; after transsphenoidal hypophysectomy or after other surgical intervention that affects the dura mater; simultaneous reception of MAO inhibitors or within 2 weeks after their cancellation; children’s age up to 6 years. Active ingredient: oxymetazoline hydrochloride 0.5 mg/ml. Excipients: camphor (racemic camphor), menthol (levomenthol), eucalyptol (cineol), benzalkonium chloride, polysorbate 80, propylene glycol, sodium dihydrogen phosphate dihydrate, disodium phosphate dihydrate, disodium edetate dihydrate, concentrated hydrochloric acid, purified water. OverdoseSymptoms After excessive administration of the drug into the nose or accidental ingestion of the contents of the vial, symptoms of both stimulation and inhibition of the central nervous system (CNS), cardiovascular and pulmonary systems can be observed. CNS stimulation is manifested by the following symptoms: anxiety, agitation, hallucinations, convulsions. Symptoms of inhibition: decrease in body temperature, lethargy, drowsiness and coma. The following additional symptoms may be noted: miosis, mydriasis, fever, sweating, pallor, cyanosis, palpitations, tachycardia, bradycardia, arrhythmia, cardiac arrest, arterial hypertension, arterial hypotension, pulmonary edema, nausea and vomiting, respiratory depression and apnea, mental disorders. In case of overdose in children, CNS disorders often occur, accompanied by convulsions, bradycardia, arterial hypertension, hypotension up to shock, apnea and coma, hallucinations. Treatment In case of severe overdose, intensive therapy is carried out. Apply activated charcoal (adsorbent), sodium sulfate (laxative) or magnesium, gastric lavage to reduce the absorption of oxymetazoline. Vasopressors are contraindicated. As an antidote, non-selective a-blockers are indicated. If necessary: temperature reduction, anticonvulsant therapy and artificial ventilation of the lungs. Side effects Oxymetazoline is generally well tolerated and side effects, if they occur, are mild and temporary. Often: burning or dryness of the nasal membranes, sneezing. Infrequently: after the effect of the drug has passed, a strong feeling of “congestion” of the nose (reactive hyperemia), epistaxis, allergic reactions (angioedema, urticaria, itching). Rarely: palpitations, tachycardia, arterial hypertension. Very rare: Infantile apnea, insomnia, fatigue (drowsiness, sedation), headache, restlessness, hallucinations (mainly in children), arrhythmia, convulsions (especially in children). Not known: dizziness, lightheadedness, trembling, nervousness, agitation, anxiety, blurred vision, increased nasal discharge, increased nasal congestion, dry mouth, irritation of the nose and throat, nausea, rash, weakness.
