It has an effect on the central nervous system in various ways: it changes the rate of propagation of excitation in the brain, improves metabolic conditions for neuronal plasticity. It improves microcirculation by affecting the rheological characteristics of the blood and does not cause a vasodilating effect. Piracetam helps to restore cognitive abilities after cerebral dysfunction due to hypoxia, intoxication or trauma. Reduces the severity and duration of vestibular nystagmus. Piracetam is used in the treatment of cortical myoclonus as monotherapy or in combination with antimyoclonic agents. Indications Piracetam is indicated for the symptomatic treatment of memory or intellectual impairment in the absence of a diagnosis of dementia (symptomatic treatment of chronic psychoorganic syndrome). Method of administration and doses In the treatment of chronic conditions, piracetam is used orally (before meals), washed down with plenty of water (100–200 ml), starting with 1200 mg (400 mg 3 times a day), and adjusting the dose to 2400 mg, sometimes up to 3200 mg or more per day. The therapeutic effect in these cases is noted, as a rule, 2-3 weeks after the start of treatment. Subsequently, the dose is reduced to 1200-1600 mg (400 mg 3-4 times a day). The course of treatment with piracetam lasts from 2–3 weeks to 2–6 months. If necessary, it is repeated after 6-8 weeks. In the symptomatic treatment of chronic psychoorganic syndrome, depending on the severity of symptoms, 1200-2400 mg / day is prescribed, and during the first week – 4800 mg / day. With cortical myoclonus, treatment begins with 7200 mg / day. If necessary, every 3-4 days, the dose is increased by 4800 mg / day until a maximum dose of 24,000 mg / day is reached in 2-3 doses. In the treatment of cortical myoclonus, it is recommended to use another drug with a high content of piracetam. A daily dose of 24,000 mg of piracetam is prescribed daily for 3 days. In case of low response or no response on the third day of treatment, the appointment of piracetam is extended to 7 days. In case of low response or no response on the seventh day of therapy, treatment should be discontinued. If a daily dose of 24,000 mg has been effective, begin to reduce the dose by 1200 mg every 2 days until the onset of myoclonus. This scheme allows you to choose the average therapeutic dose of piracetam. During the selection of the dose of piracetam, changing the doses of other antimyoclonic agents is not carried out. In the treatment of cortical myoclonus, treatment is continued throughout the entire period of the disease.
