Magnesium reduces the excitability of neurons, participates in neuromuscular synaptic transmission, muscle contraction and regulation of enzymatic processes. Magnesium is a macronutrient; half of the magnesium contained in the body accumulates in bone tissue. Clinical aspects: Serum magnesium levels: between 12 and 17 mg/l (1-1.4 mEq/l or 0.5-0.7 mmol/l): indicate a mild magnesium deficiency. below 12 mg/l (1 mEq/l or 0.5 mmol/l): indicate severe magnesium deficiency. Deficiency can be: primary, due to a congenital anomaly of metabolism (chronic congenital hypomagnesemia), secondary, due to inadequate intake (severe malnutrition, alcoholism, exclusively parenteral nutrition) , disorders of gastrointestinal absorption (chronic diarrhea, gastrointestinal fistula, hypoparathyroidism), excessive losses at the renal level (tubular disease, severe polyuria, diuretic abuse, chronic pyelonephritis, primary hyperaldosteronism, cisplatin treatment). Pyridoxine (vitamin B6) is involved in many metabolic processes, including those in the nervous system. Vitamin B6 improves the absorption of magnesium from the gastrointestinal tract and its penetration into cells. Pharmacokinetics Gastrointestinal absorption of magnesium salts occurs in part by a passive mechanism in which salt solubility plays a decisive role. The degree of this absorption does not exceed 50%. Excretion occurs primarily in the urine. Indications for use This medicinal product contains magnesium and is used to correct the deficiency of this element in the body. A combination of several of the following symptoms may indicate a magnesium deficiency: nervousness, irritability, mild anxiety, transient fatigue, slight sleep disturbances, signs of anxiety such as gastrointestinal cramps or heart palpitations (with a healthy heart), muscle cramps, tingling sensation. magnesium can help relieve these symptoms. If after one month of treatment there is no improvement, continuation of monotherapy with this drug is impractical. Method of administration and doses The use of this medicinal product is possible only after consulting a doctor! The tablets should be taken whole with a glass of water. Adults: 3-4 tablets of 100 mg/10 mg or 6-8 tablets of 50 mg/5 mg per day, divided into 2-3 doses, with meals. Children over the age of 6 years (weighing about 20 kg): 10-30 mg / kg / day (0.4-1.2 mmol / kg / day), i.e. 2-4 tablets at a dosage of 100 mg / 10 mg or 4-6 tablets at a dosage of 50 mg / 5 mg per day, divided into 2-3 doses, with meals. Usually the duration of treatment is one month. The notch on the 100 mg/10 mg tablets is intended solely to facilitate taking one tablet (by breaking the tablet into two halves) and not to divide the tablet into two doses. Treatment should be discontinued when serum magnesium levels return to normal. If you forget to take a medicine, do not take a double dose to make up for the missed one! Use during pregnancy and lactation If you are pregnant or breastfeeding, if you suspect that you are pregnant or do not exclude the possibility of pregnancy, inform your doctor. Clinical experience, in a sufficient number of pregnancies, has not revealed any fetotoxic or developmental defect-causing actions. If necessary, magnesium can be used at any stage of pregnancy. Despite the lack of large-scale studies, the available clinical experience is encouraging. The use of magnesium and vitamin B6 contained in the preparation is considered compatible with lactation. The recommended dose of vitamin B6 during lactation is no more than 20 mg per day. Precautions Due to the presence of lactose in the tablets, the drug is contraindicated in congenital galactosemia, glucose or galactose absorption disorder, and lactase deficiency. The tablets are intended for adults and children over 6 years of age. In moderate renal insufficiency, care should be taken to prevent the risk associated with hypermagnesemia. When pyridoxine is used at high doses (>200 mg/day) for a long time (for several months or in some cases years), sensory neuropathy may develop, which is accompanied by symptoms such as numbness and vestibular disturbances, tremor of the distal extremities and gradually developing sensory ataxia (impaired coordination of movements). These disorders are usually reversible and disappear after the abolition of high doses of vitamin B6. Interaction with other drugs Not recommended combinations: Phosphate or calcium salts – these drugs inhibit the absorption of magnesium from the intestine. Contraindicated combinations: Levodopa – the activity of levodopa (if this drug is not combined with the use of peripheral dopa decarboxylase inhibitors) is inhibited by pyridoxine. Any intake of pyridoxine should be avoided unless peripheral dopa-decarboxylase inhibitors are prescribed concomitantly with levodopa. Combinations to be taken into account: Orally administered tetracyclines – an interval of at least 2 hours should be observed between oral tetracyclines and magnesium preparations, due to a decrease in absorption tetracycline in the gastrointestinal tract. Composition Each tablet contains: Active ingredients: magnesium (in the form of anhydrous magnesium citrate) – 100.0 mg; pyridoxine hydrochloride – 10.0 mg. Excipients: macrogol (6000), magnesium stearate, microcrystalline cellulose, lactose monohydrate, Opadry II white (polyvinyl alcohol, titanium dioxide, polyethylene glycol, talc). Overdose Oral overdose of magnesium usually does not lead to toxic reactions if the kidneys are functioning normally. However, magnesium poisoning can develop in case of kidney failure. Toxic effects depend on the level of magnesium in the blood. Symptoms: drop in blood pressure; nausea, vomiting; CNS depression, impaired reflexes; ECG anomalies; respiratory depression, coma, cardiac arrest and respiratory paralysis; anuric syndrome. Treatment: rehydration, forced diuresis. Renal failure requires hemodialysis or peritoneal dialysis.
