Pharmacotherapeutic group Multivitamins with minerals. Pharmacological properties Pharmacodynamics. The drug contains in its composition a combination of vitamins with calcium and phosphorus. Vitamins are substances that take part in the regulation of many biochemical processes in the body. The action of vitamins and minerals in the body is more physiological than pharmacodynamic. The B group of vitamins (B1, B2, B6, B12, pantothenic acid, nicotinamide and folic acid) is involved in the metabolism of carbohydrates, proteins and fats, and also plays an important role in the functioning of the nervous system. Folic acid is necessary for the formation, regeneration and normal functioning of blood cells. Vitamin A is necessary for the development of epithelial cells and the synthesis of visual pigment. Vitamin D regulates calcium absorption and enables proper mineralization of bones and teeth. Vitamin C promotes the absorption of iron and is involved in many redox processes in the body. Minerals are components of connective tissue and act as activators and constituents of enzymes. Calcium and phosphorus play an important role in the mineralization of bones and teeth. Calcium ions activate many enzymes, take part in the regulation of the tone of the heart muscle, in the regulation of the transmission of nerve impulses and regulate the permeability of cell membranes. Pharmacokinetics. Water-soluble vitamins (vitamins of the B group, vitamin C and biotin) are well absorbed in amounts corresponding to the daily requirement. Excessive amounts are excreted in the urine and in some cases also in the faeces. These vitamins are stored in the body in limited quantities, so they must be regularly supplied with food to maintain the required concentration in the tissues. In the presence of fats, orally ingested fat-soluble vitamins A and D are well absorbed in the small intestine. When used in large quantities, these vitamins are stored and deposited in the liver, and therefore are more toxic than water-soluble vitamins. Indications Prevention and treatment of vitamin and mineral deficiency in children with inadequate and unbalanced nutrition; in conditions that are accompanied by an increased need for vitamins and minerals (growth period, illness, recovery period, in complex treatment with antibiotic therapy); with increased physical and neuropsychic stress. Contraindications Hypersensitivity to the active substances or to any of the excipients of the medicinal product; hypervitaminosis A and D; nephrolithiasis; gout; hyperuricemia; erythremia; erythrocytosis; thrombophlebitis; thromboembolism; violation of the metabolism of iron and copper; hypercalcemia; hypercalciuria; thyrotoxicosis; chronic glomerulonephritis; chronic heart failure; history of sarcoidosis; ulcer of the stomach and duodenum (due to the possibility of increasing the acidity of gastric juice); tuberculosis; children under 4 years old. Interaction with other drugs In connection with the possibility of developing hypervitaminosis A, the concomitant use of the drug with other drugs containing vitamin A, or with oral retinoids is not recommended. When treating with sulfonamides, high doses of vitamin C should be avoided to prevent crystalluria. Pyridoxine reduces the effectiveness of levodopa if the patient is on levodopa alone. Riboflavin interferes with absorption and reduces the effectiveness of antibiotics. Therefore, it should be used at least 3 hours before antibiotic use. If you are taking any other medicines, consult your doctor about the possibility of using the drug. Features of the applicationBefore starting treatment, consult your doctor! The drug is not recommended for children under 4 years of age. It is not recommended to use with other vitamin preparations in order to avoid overdose and the development of side effects. Taking vitamins does not replace a balanced diet. Do not exceed the indicated dosage. High doses of certain components, especially vitamin A, vitamin D, can be hazardous to health. If hypersensitivity reactions to the components of the drug occur, the drug should be discontinued. Absorption of ascorbic acid can be disturbed with intestinal dyskinesias, enteritis and achilia. Since ascorbic acid increases iron absorption, its use in high doses can be dangerous for patients with hemochromatosis, thalassemia, polycythemia, leukemia, and sideroblastic anemia. Ascorbic acid as a reducing agent can affect the results of laboratory tests, for example, when determining the content of glucose, bilirubin, transaminase activity, lactate dehydrogenase. Use with caution in patients with liver disease, acute nephritis, cardiac decompensation, cholelithiasis, chronic pancreatitis, allergic diseases, idiosyncrasy, neoplasms. It is possible to stain urine yellow due to the content of riboflavin. This effect is safe. The drug contains lactose, so it should not be used in patients with rare hereditary forms of galactose intolerance, lactase deficiency or glucose-galactose malabsorption syndrome. Since the medicinal product contains sorbitol, patients with rare hereditary fructose intolerance should not use this medicinal product. The drug contains aspartame, which is converted to phenylalanine in the body, so it should not be used in patients with phenylketonuria. The medicinal product contains sugar, which should be taken into account in patients with diabetes mellitus. Use during pregnancy or lactation Use the drug in children. Ability to influence the rate of reactions when driving vehicles or other mechanisms There are no reports on the effect of the drug on the ability to drive a car or work with other mechanical means. Children Use the drug in children from 4 years of age. Dosage and administration Take the drug after meals. Children 4-6 years old take tablets only in the presence of adults. In the absence of the skill of taking pills, this dosage form is unsuitable for children. For the prevention of hypovitaminosis Children from 4 to 6 years old – 1 tablet 1-2 times a day; Children from 7 to 14 years old – 1 tablet 2-3 times a day. For the treatment of hypovitaminosis Children from 4 to 6 years old – 1 tablet 4 times a day; Children from 7 to 14 years old – 1 tablet 5 times a day. With reduced appetite, the drug Kievit® is recommended to be used for 2 months, for other indications – if necessary, depending on the duration and frequency of symptoms, after consulting a doctor. Overdose When using the recommended doses, no intoxication is expected. Symptoms. Long-term use (more than 1 month) of very large doses of the drug can lead to hypervitaminosis A and D. However, the possibility of overdose when using the drug is unlikely. Treatment: symptomatic. Side effects From the immune system: hypersensitivity reactions to the components of the drug are possible, including anaphylactic shock, bronchospasm, Quincke’s edema. From the side of metabolism and digestion: hypercalcemia. From the nervous system: headache, dizziness, drowsiness. On the part of the organs of vision: blurred vision. From the gastrointestinal system: dyspepsia, nausea, vomiting, belching, stomach pain, constipation, diarrhea, increased secretion of gastric juice. From the skin and subcutaneous tissue: rash, urticaria, itching, redness. From the side of the kidneys and urinary tract: discoloration of urine, hypercalciuria. General disorders: hyperthermia, irritability, hyperhidrosis. During long-term use of high doses, the following adverse reactions may occur. From the side of metabolism and digestion: hyperuricemia, impaired glucose tolerance, hyperglycemia. From the nervous system: paresthesia. From the side of the cardiovascular system: arrhythmias. From the gastrointestinal tract: gastrointestinal disorders. From the skin and subcutaneous tissue: hair loss, seborrhea. From the side of the kidneys and urinary tract: renal failure. Laboratory indicators: a temporary increase in aspartate aminotransferase, alkaline phosphatase, lactate dehydrogenase.
