Pharmacological properties The properties of the medicinal product – “Lorinden A” ointment – result from the combined effect of flumethasone pivalate and salicylic acid. Flumethasone pivalate is a synthetic corticosteroid with a moderately strong anti-inflammatory effect. When applied topically, it has an anti-inflammatory, antipruritic and constrictive effect on blood vessels. Salicylic acid in the concentration present in the preparation exhibits weak keratolytic activity. Facilitates the penetration of glucocorticosteroid through the stratum corneum of the epidermis. Flumethasone pivalate, after application to the skin, easily penetrates into the stratum corneum of the skin. To a small extent, it can penetrate the body and then has a general effect. After absorption through the skin of flumethasone, pivalate is metabolized mainly in the liver. It is then eliminated in the urine and, to a lesser extent, in the bile, mainly in the form of compounds with glucuronic acid and in small amounts unchanged. The absorption of flumethasone pivalate through the skin increases after application to delicate skin in the area of u200bu200bfolds or on the skin of the face, as well as on skin with damaged epidermis or damaged as a result of an inflammatory process. The absorption of flumethasone pivalate increases after its application to large areas of the skin. Absorption of flumethasone pivalate through the skin in children is always higher than in adults. Indications for use Dry inflammatory processes of the skin, especially of allergic etiology, treatable with corticosteroids, uncomplicated by bacterial infection and proceeding with excessive keratinization and persistent itching. Ointment “Lorinden A” is mainly used for: – seborrheic dermatitis, – atopic dermatitis, – papular urticaria, – allergic contact dermatitis, – erythema multiforme exudative, – systemic lupus erythematosus, – chronic psoriasis, – lichen planus. Dosage and administration Do not use more than once or twice a day. Do not use the drug for more than 2 weeks. Do not use on the face for more than 7 days. Within a week, you can use no more than 15 g (1 tube) of ointment. Children and adolescents Do not use in children under 2 years of age. In children older than 2 years, use with extreme caution, only once a day, on a small area of the skin, do not use on the face. How to use Apply to the skin. A thin layer of ointment should be applied to pathologically altered skin. In case of excessive manifestations of lichen or excessive keratinization of pathologically altered skin, an occlusive dressing is allowed, which should be changed every 24 hours. Application during pregnancy and lactation Pregnancy “Lorinden A” can be used in pregnant women only for a short time, on a small surface of the skin, if the benefit to the mother outweighs the risk to the fetus. Do not use in the first trimester of pregnancy. Breast-feeding The degree of release of flumethasone pivalate into breast milk when applied externally is not known. It is recommended that Lorinden A ointment be used with caution in women during breastfeeding only if the benefit to the mother is higher than the potential risk to the baby, for a short time and on small areas of the skin. Do not apply Lorinden A ointment to the skin of the mammary glands. Precautions If irritation or excessive drying of the skin occurs during the use of the medicinal product, the use of the product should be discontinued immediately. Do not apply continuously for more than 2 weeks. Corticosteroids are absorbed through the skin. Therefore, during the use of the medicinal product, there is a risk of systemic negative symptoms of corticosteroids (including inhibition of the functions of the adrenal cortex, a decrease in the concentration of cortisol in the blood, Cushing’s syndrome). Based on these considerations, the use of the medicinal product on large areas of the skin, wounds, damaged skin, the use of large doses, as well as prolonged treatment with the product should be avoided. During treatment, periodic monitoring of the functions of the adrenal cortex is indicated by determining cortisol in the blood and urine after stimulation of the adrenal glands with ACTH /adrenocorticotropic hormone/. In case of skin infection at the treatment site, additional antibacterial or antifungal treatment should be applied. If the symptoms of the infection persist, the medicinal product should be discontinued until the infection has resolved. The medicinal product should not be used on the eyelids or on the skin around the eyelids due to the risk of glaucoma or cataracts, and in people with glaucoma or cataracts, as the symptoms of these diseases may worsen. Avoid getting the drug in the eyes, open wounds and mucous membranes. If the drug gets into the eyes or mucous membranes, it is recommended to rinse them with plenty of water. As in the case of the use of systemic glucocorticosteroids, after external use of the drug, cataracts and glaucoma or such a rare disease as central serous chorioretinopathy (CSC) may develop. Consult your doctor if you experience blurred vision or other visual disturbances. On the skin of the face and on the skin of the armpits and groin, it should be used only in cases of emergency, since the absorption of the corticosteroid through delicate skin is more intense, and this is associated with an increased risk of telangiectasia, oral dermatitis, cutaneous dystrophy, even after short use. Avoid using the product under an occlusive dressing (oilcloth, diaper), as the dressing enhances the absorption of corticosteroid by the body, in addition, epidermal dystrophy, stretch marks and secondary infections may occur. Use with caution in conditions of subcutaneous tissue dystrophy, especially in the elderly. Use with extreme caution in patients with psoriasis, since topical corticosteroids in psoriasis can be dangerous for many reasons, including due to relapses of the disease caused by the development of addiction, the risk of generalized pustular psoriasis (von Zumbusch pustular psoriasis) and general toxic effects. caused by a violation of the integrity of the skin. Children and adolescents Use with caution in children over 2 years of age. In children, due to the greater proportion of body area in relation to body weight than in adults, it is easier than in adults to inhibit the functions of the hypothalamus-pituitary-adrenal axis and the appearance of undesirable symptoms characteristic of corticosteroids, including .disturbances of growth and development. Due to the content of propylene glycol, the medicinal product may cause skin irritation. Due to the content of lanolin, the medicinal product may cause local skin reactions (eg contact dermatitis). Interaction with other drugs No interaction studies have been conducted so far. However, it should be borne in mind that patients should not be vaccinated against smallpox or other methods of immunization, especially with prolonged use of the medicinal product on a large area of the skin, due to the risk of an abnormal immune response. In the case of prolonged use of the medicinal product on large areas of the skin, as a result of the absorption of salicylic acid by the body, there may be an increase in the effects of methotrexate, oral antidiabetic drugs, sulfonylurea derivatives. Ointment “Lorinden A” can enhance the effect of immunosuppressive drugs and weaken the effect of immunostimulating drugs. Contraindications Do not use the drug in case of: – hypersensitivity to flumethasone pivolat, other corticosteroids, salicylic acid or any of the excipients of the medicinal product, – viral (for example, chickenpox, herpes vulgaris), fungal or bacterial skin infections, – skin neoplasms, – acne , – telangiectasia, – periorbital dermatitis, – rosacea, – anal itching, itching in the vulva, – diaper dermatitis, – inflammation or trophic ulcer, – with extensive skin changes, especially if the integrity of the skin is violated, – in children under 2 years old. Excipients: propylene glycol, white petrolatum, anhydrous lanolin. Overdose Prolonged or incorrect topical application of the medicinal product or its use on large areas of the skin can lead to inhibition of the functions of the hypothalamus-pituitary-adrenal axis, inhibition of growth and development in children. Symptoms of an overdose may occur in the form of edema, increased pressure, hyperglycemia, sugar in the urine, a decrease in body resistance, and in severe cases, Cushing’s disease. In this case, the medicinal product should be gradually discontinued or replaced with a less potent corticosteroid. Rarely, symptoms of salicylate poisoning may occur. Side effects Skin and subcutaneous tissue disorders Symptoms similar to seborrhea, post-steroid spotted pigmentation, degeneration of the epidermis and subcutaneous tissue, dry skin, excessive hair growth or baldness, depigmentation or hyperpigmentation of the skin, inflammation of the hair follicles, secondary infections, skin irritations may occur. Undesirable systemic symptoms of flumethasone pivalate are characteristic of corticosteroids, among other things, inhibition of the functions of the hypothalamic-pituitary-adrenal axis, Cushing’s syndrome, inhibition of growth and development in children, hyperglycemia, sugar in the urine, swelling, high blood pressure, decreased immunity.
