Use in pregnancy and lactation Pregnancy Low doses (up to 100 mg/day) Clinical studies show that doses up to 100 mg/day are safe for use under certain conditions in pregnant women requiring some monitoring. Fertility: Acetylsalicylic acid is not recommended for women planning a pregnancy, because. inhibition of prostaglandin synthesis reduces fertility. The effect on fertility is reversible. If aspirin treatment is unavoidable, it should be as short as possible and at low doses. Suppression of prostaglandin synthesis can adversely affect pregnancy and / or intrauterine development of the embryo. Data from epidemiological studies show an increased risk of spontaneous abortion and congenital heart defects of the child and gasroschisis after the use of prostaglandin synthesis inhibitors in early pregnancy. The absolute risk of congenital heart disease increases from less than 1% to 1.5%, which is presumably associated with an increase in the dose and duration of treatment. With the combined appointment of glucocorticosteroids (GCS) and salicylates, it should be remembered that during treatment, the level of salicylates in the blood is reduced, and after the abolition of GCS, an overdose of salicylates is possible. The combination of acetylsalicylic acid with ibuprofen is not recommended, since the latter worsens the beneficial effect of acetylsalicylic acid on life expectancy. Exceeding the dose of acetylsalicylic acid is associated with a risk of gastrointestinal bleeding. Overdose is especially dangerous in elderly patients. When acetylsalicylic acid is combined with alcohol, the risk of damage to the gastrointestinal mucosa and prolongation of bleeding time is increased. Interaction with other drugs When used simultaneously, acetylsalicylic acid enhances the effect of the following drugs: methotrexate (by reducing renal clearance and displacing it from its association with proteins); heparin and indirect anticoagulants (due to impaired platelet function and displacement of indirect anticoagulants from their association with proteins); thrombolytic and antiplatelet drugs (ticlopidine), digoxin (due to a decrease in its renal excretion); hypoglycemic agents: insulin and sulfonylurea derivatives (due to the hypoglycemic properties of acetylsalicylic acid itself in high doses and the displacement of sulfonylurea derivatives from association with proteins); valproic acid (by displacing it from its association with proteins). An additive effect is observed while taking acetylsalicylic acid with ethanol (alcohol). Acetylsalicylic acid weakens the effect of uricosuric agents (benzbromarone) due to competitive tubular elimination of uric acid. By enhancing the elimination of salicylates, systemic glucocorticosteroids weaken their effect. Antacids and cholestyramine, when used simultaneously, reduce the absorption of Cardiomagnum. Overdose Symptoms of an overdose of moderate severity: nausea, vomiting, tinnitus, hearing loss, dizziness, confusion. Treatment: dose reduction of the drug. Symptoms of severe overdose: fever, hyperventilation, ketoacidosis, respiratory alkalosis, coma, cardiovascular and respiratory failure, severe hypoglycemia. Treatment: immediate hospitalization in specialized departments for emergency therapy – gastric lavage, determination of acid-base balance, alkaline and forced alkaline diuresis, hemodialysis, administration of saline solutions, activated charcoal, symptomatic therapy. When performing alkaline diuresis, a pH value between 7.5 and 8 should be achieved. Forced alkaline diuresis should be performed when the plasma salicylate concentration is more than 500 mg/l (3.6 mmol/l) in adults and 300 mg/l (2.2 mmol/l) in children.
