Composition of the drug active ingredient: dipyridamole; 1 tablet contains dipyridamole (in terms of 100% dry matter) – 25 mg; excipients: potato starch, lactose monohydrate, calcium stearate. Pharmacotherapeutic group Antithrombotic agents. Antiplatelet agents. Dipyridamole. ATC code: B01A C07. Indications for use The drug is used as an adjunct to oral anticoagulants for the prevention of thromboembolism after heart valve replacement surgery. Contraindications Hypersensitivity to dipyridamole or other components of the drug; acute myocardial infarction; unstable angina; decompensated heart failure; severe cardiac arrhythmias; severe arterial hypotension (systolic pressure below 90 mm Hg. Art.); collapse; hemorrhagic diathesis, diseases with a tendency to bleeding (for example, peptic ulcer of the stomach and duodenum); severe renal or hepatic insufficiency; widespread stenosing atherosclerosis of the coronary arteries; subaortic stenosis of the aorta; bronchial asthma, obstructive pulmonary disease; children’s age up to 18 years; pregnancy and breastfeeding. Precautions Be wary of prescribing to patients: with arterial hypotension; with severe pathology of the coronary arteries (for example, a recent myocardial infarction, heart failure) – chest pain may increase due to the vasodilatory effect; the appointment of large doses of the drug can cause the “steal” syndrome (decrease in blood supply to ischemic areas of the myocardium); with myasthenia gravis (risk of exacerbation) – dose adjustment of dipyridamole may be required; with the simultaneous use of drugs that increase the risk of bleeding. The use of dipyridamole can lead to an exacerbation of migraine. The use of high doses of dipyridamole can lead to an increase in the level of liver enzymes in the blood plasma and liver failure. There have been cases of inclusion of unconjugated dipyridamole in gallstones (up to 70% of the dry weight of the calculus) in elderly patients with signs of cholangitis when taking dipyridamole for several years. There are no data on the use of the drug in elderly patients, with hepatic or renal insufficiency, therefore, in these cases, it should be used with caution. During treatment, you should refrain from frequent use of strong coffee or tea. The drug should not be used in patients with lactase deficiency, galactosemia, glucose-galactose malabsorption syndrome (the drug contains lactose). Use during pregnancy or lactation Do not use the drug during pregnancy. The drug passes into breast milk, so breast-feeding should be discontinued for the duration of the drug. Children There are no data on the efficacy and safety of the use of the drug Dipyridamole in pediatric practice, therefore, it is not recommended for use in children under 18 years of age. The ability to influence the reaction rate when driving vehicles or working with other mechanisms A possible decrease in blood pressure and the resulting lack of blood supply to the brain can affect the ability to drive vehicles or work with potentially dangerous mechanisms, so care should be taken. Overdose Symptoms: general vasodilation, arterial hypotension, increased sweating, headache, irritability, angina attacks, tachycardia, feeling of warmth in the body, reddening of the skin of the face, weakness, dizziness. Treatment: symptomatic therapy: gastric lavage, induction of vomiting, measures to reduce the absorption of the drug. Slow intravenous administration of xanthine derivatives (for example, 50-100 mg of aminophylline for 1 minute) blocks the vasodilating effect of dipyridamole. To eliminate arterial hypotension, the introduction of vasopressor agents, especially mezaton at a dose of 0.5-1 ml intramuscularly, is indicated. In case of angina attacks, apply nitroglycerin under the tongue. Since dipyridamole is highly bound to plasma proteins, dialysis is not effective. Side effectsImmune system: allergic reactions, hypersensitivity reactions, including itching, skin rashes, urticaria, bronchospasm, angioedema. Nervous system: headache, dizziness of vascular origin, weakness, fainting, tremor. Cardiovascular system: chest pain, palpitations, exacerbation of symptoms of coronary heart disease, such as angina pectoris, arrhythmia (including tachycardia, bradycardia), myocardial infarction, arterial hypotension, feeling hot, flushing of the skin of the face (hot flashes), coronary steal syndrome. Digestive tract: dyspepsia, epigastric discomfort/pain, nausea, vomiting, diarrhea. Hepatobiliary system: increased levels of liver enzymes; there have been reports of dipyridamole as an ingredient in gallstones. Others: myalgia, arthritis, increased bleeding during or after surgery, bleeding, thrombocytopenia, changes in the functional properties of platelets. Shelf life 4 years.
