DescriptionCapsules hard gelatin No. 0 cylindrical shape with hemispherical ends; body white, cap orange. The contents of the capsules are an oily mass from yellow to orange. The presence of particles from yellow-white to brown is allowed. Pharmacological action Combined drug, the action of which is due to the properties of its constituent components. “Essential phospholipids” are complex substances of natural origin, which are diglyceride esters of phosphatidic and unsaturated fatty acids (mainly oleic and linolenic). They are an important structural element of cell membranes and membranes of hepatocyte organelles. Phospholipids regulate membrane permeability, the activity of membrane-bound enzymes, ensuring normal processes of oxidative phosphorylation. The drug normalizes the biosynthesis of phospholipids and the overall lipid metabolism in case of damage to hepatocytes of various etiologies. Restores hepatocyte membranes through structural regeneration and through competitive inhibition of oxidative processes: by integrating into biomembranes, unsaturated fatty acids are able to take on toxinogenic effects instead of membrane lipids. Promotes the regeneration of liver cells, stabilizes the physicochemical properties of bile. Thiamine mononitrate (vitamin B1) as a coenzyme is involved in carbohydrate metabolism. Riboflavin (vitamin B2) is the most important catalyst for cellular respiration processes. Pyridoxine hydrochloride (vitamin B6) as a coenzyme takes part in the metabolism of amino acids and proteins. Cyanocobalamin (vitamin B12) together with folic acid is involved in the synthesis of nucleotides. Nicotinamide is involved in the processes of tissue respiration, fat and carbohydrate metabolism. Alpha-tocopheryl acetate (vitamin E) has antioxidant properties, protects unsaturated fatty acids in membranes from lipoprotection. Pharmacokinetics When taken orally, more than 90% of phospholipids are absorbed from the gastrointestinal tract. The main amount is broken down to 1-acyl-LYSO-phosphatidylcholine, 50% of which is converted into polyunsaturated phosphatidylcholine even in the small intestine. Further, polyunsaturated phosphatidylcholine in combination with HDL is transported to the liver. The maximum concentration of phospholipids in the blood plasma is reached after 6 hours. Metabolism occurs in the kidneys and liver. Excreted from the body with feces and, to a lesser extent, with urine. Indications for use Disorders of lipid metabolism of various origins; acute and chronic hepatitis; cirrhosis of the liver; alcohol and drug intoxication; poisoning (including medicinal); dysfunction of hepatocytes (as a complication in other diseases); Radiation damage to the liver psoriasis (as part of combination therapy). Contraindications Hypersensitivity to the components of the drug, cholestasis, gallbladder empyema, nephrolithiasis, peptic ulcer of the stomach and duodenum in the acute stage. Pregnancy and lactation Data on the safety and efficacy of the drug during pregnancy and lactation are not presented. Dosage and administration Inside, during meals; Capsules should be swallowed whole with plenty of water. The drug is prescribed 2 capsules 2-3 times a day. The duration of the course of treatment is at least 5 months; if necessary, longer use of the drug or repeated courses of treatment is possible. In psoriasis, the drug is used as an adjuvant therapy, 2 capsules 3 times a day for 2 weeks. Side effectsUsually the drug is well tolerated. Very rarely, flatulence, nausea, diarrhea, dark yellow urine, discomfort in the stomach, hypersensitivity reactions, skin rashes, urticaria, itching are possible. Overdose Currently, cases of overdose are not known. Interaction with other medicinal products Multivitamin medicinal products: The vitamin content of Essel Forte E should be taken into account to prevent vitamin overdoses. Tocopherol acetate enhances the effect of steroidal and non-steroidal anti-inflammatory drugs (sodium diclofenac, ibuprofen, prednisone). Tocopherol acetate should not be taken with iron, silver preparations; with agents that exhibit an alkaline reaction (sodium bicarbonate, trometamol), indirect anticoagulants (dicoumarin, neodicoumarin). Pyridoxine can weaken the effect of levodopa and reduce the toxic effects of isoniazid and other anti-tuberculosis drugs. Pyridoxine hydrochloride enhances the action of diuretics. Isoniazid, penicillamine, pyrazinamide, hydralazine, immunosuppressants, estrogens (including oral contraceptives containing estrogens) act as pyridoxine antagonists or increase renal excretion of the latter. Riboflavin is incompatible with streptomycin, reduces the effectiveness of antibacterial drugs (oxytetracycline, doxycycline, erythromycin, tetracycline, lincomycin). The tricyclic antidepressants imipramine and amitriptyline inhibit the metabolism of riboflavin. Thiamine can weaken the curare-like effect. Antacids slow down the absorption of thiamine. The action of thiamine is inactivated by 5-fluorouracil, since the latter competitively inhibits the phosphorylation of thiamine to thiamine pyrophosphate. Cholestyramine: weakens the absorption of nicotinamide; an interval of 4-6 hours between taking these medicines is recommended. Rifampicin: decreased bilirubin synthesis and increased protein synthesis. Paracetamol: decrease in serum transaminase levels. Precautions The use of the drug requires caution in such cases: in patients with allergic diseases, with a history of gastric and duodenal ulcers, with diseases of the heart and blood. In the event of an allergic reaction to the drug, you should stop taking it. For patients with chronic gastritis, to facilitate the digestion of gelatin capsules, the additional use of products containing digestive enzymes is recommended. Children. The drug is used to treat children over 12 years of age.
