The main active ingredient Dimenhydrinate Release form Tablets Dosage 50 mg Pharmacological properties Pharmacodynamics Dimenhydrinate belongs to ethanolamine derivatives, it contains 53–55.5% diphenhydramine and 44–47% 8-chlorotheophylline. Dimenhydrinate is an H1 receptor blocker, has antiemetic, sedative, antimuscarinic, antihistamine effects. Reduces the excitability of the vomiting center of the central nervous system. Pharmacokinetics After oral administration, dimenhydrinate is absorbed. The maximum concentration in blood plasma is reached in 15-30 minutes. after administration and persists for 4-6 hours. Approximately 98-99% of dimenhydrinate binds to plasma proteins. Dimenhydrinate crosses the placental barrier and is excreted in breast milk. The metabolism of dimenhydrinate is carried out mainly in the liver. It is excreted from the body within 24 hours, mainly in the form of metabolites, in a smaller amount – unchanged by the kidneys. The half-life in plasma is almost 5-10 hours. Indications for use Prevention and treatment of nausea, vomiting in sea and air sickness. Prevention and treatment of symptoms of vestibular and labyrinth disorders (dizziness, nausea, vomiting), with the exception of symptoms caused by anticancer therapy. Meniere’s disease. Route of administration and doses Prevention of motion sickness in transport The drug is taken at least 30 minutes before the planned trip. Recommended dosages: adults and children over 14 years old: 50-100 mg (1-2 tablets); children aged 5-14 years: 25-50 mg (1/2-1 tablet). If necessary, the indicated dose can be taken again with intervals every 4-6 hours. Nausea and vomiting Adults and children over 14 years of age: 50–100 mg (1–2 tablets) every 4–6 hours as needed, up to a maximum of 400 mg per day. Children 5–14 years of age: 25–50 mg (1/2–1 tablet) every 6–8 hours as needed, up to a maximum of 150 mg per day. The maximum daily dose is 5 mg per 1 kg of body weight. Elderly people should use the lowest dose recommended for adults, as they are sensitive to anticholinergic effects. The duration of the drug in adults and children over 12 years old is 2-3 days; in children 5-12 years old – 1 day. The medicine can be taken regardless of food intake. Tablets should be swallowed with plenty of liquid (eg water). For children under 6 years of age, it is advisable to crush the required dose of the tablet before taking it to avoid the risk of inhalation. Use during pregnancy and during breastfeeding During pregnancy, the drug is contraindicated. Dimenhydrinate passes into breast milk in small amounts, so breastfeeding should be discontinued during treatment. The ability to influence the reaction rate when driving vehicles and operating other mechanisms Dimenhydrinate intake can cause drowsiness, depression, dizziness, an increase in reaction time, therefore, when using the drug, you should refrain from driving vehicles or working with mechanisms with an increased risk of injury. Precautions Dimenhydrinate should be used with caution in patients with a tendency to convulsions and in patients in whom anticholinergic drugs may worsen the course of the disease (prostatic hypertrophy, bronchial asthma). The drug can mask the symptoms of ototoxicity, so care must be taken when used simultaneously with ototoxic agents. With prolonged use, it can lead to falsely high values of theophylline in serum. Care must be taken when prescribing dimenhydrinate to patients with cardiovascular disease, hyperthyroidism, stenosing peptic ulcer, pyloroduodenal obstruction, bladder neck obstruction. Contraindications Hypersensitivity to dimenhydrinate, diphenhydramine, as well as to antihistamines of a similar structure, hypersensitivity to other components of the drug. Acute exudative vesicular dermatoses. Eclampsia, epilepsy (especially in children), renal failure, glaucoma, increased intracranial pressure. During pregnancy and breastfeeding. Children’s age up to 5 years. Composition 1 tablet contains: active substance – dimenhydrinate 50 mg. Excipients: ludipress (lactose monohydrate 93%, povidone 3.5%, crospovidone 3.5%), microcrystalline cellulose, magnesium stearate. Overdose Symptoms of overdose: dry mouth, nose and throat, redness of the face, slow and labored breathing, confusion, in children – convulsions, hallucinations. If these symptoms appear, you should immediately consult a doctor. Treatment: gastric lavage with a suspension of activated charcoal (20-30 g), the appointment of a saline laxative (10-15 g of sodium sulfate), symptomatic therapy, with convulsions in children – phenobarbital (5-6 mg / kg), diazepam. Interaction with other drugs Enhances the effects of atropine, tricyclic antidepressants, catecholamines, barbiturates, alcohol, sedatives and hypnotics, antipsychotics, weakens the effect of corticosteroids, anticoagulants. Reduces response to apomorphine. Reduces the depressive effect of acetylcholine on the heart muscle. The combination with bismuth preparations, scopolamine, painkillers and psychotropic drugs increases the likelihood of visual impairment. Incompatible with ototoxic antibiotics (streptomycin, neomycin, biomycin, amikacin, kanamycin), because it can mask the symptoms of ototoxicity and contribute to the development of irreversible hearing loss. Side effects The parameters of the frequency of occurrence of side effects: very often (? 1/10), often (? 1/100, < 1/10), infrequently (? 1/1000, < 1/100), rarely (? 1/10,000, < 1/1000), very rarely (< 1/10 000), unspecified frequency. Blood and lymphatic system disorders Very rare: hemolytic anemia, agranulocytosis, neutropenia, leukopenia, thrombocytopenia or pancytopenia. Immune system disorders Rare: anaphylactic shock. Psychiatric disorders Often: mood changes, restlessness, stiffness of movements. Uncommon: insomnia. Nervous system disorders Common: drowsiness. Infrequently: a violation of the sense of balance, impaired concentration and memory impairment (more often in elderly patients), tremor, lack of coordination, confusion, hallucinations. Rarely: headache, sleep disturbance, dizziness, orthostatic hypotension. Very rare: paradoxical stimulation of the central nervous system (especially in children). Eye disorders Rare: glaucoma, vision problems (dilated pupil, blurred vision or double vision). Hearing and labyrinth disorders Rare: tinnitus. Cardiac disorders Rare: palpitations, tachycardia and hypotension. Respiratory, thoracic and mediastinal disorders Common: Nasal congestion. Gastrointestinal disorders Common: dry mouth, constipation, diarrhea, nausea, abdominal pain. Liver and biliary tract disorders Unspecified frequency: liver dysfunction (cholestatic jaundice).
