Pharmacodynamics Butamirate, the active substance of the drug Sinekod®, is an antitussive that does not belong to opium alkaloids either chemically or pharmacologically. Sinecod® is considered to be a centrally acting antitussive. In addition to the antitussive effect, there is a trend towards a decrease in airway resistance, which is manifested in a slight increase in spirometry. Pharmacokinetics Butamirate citrate is rapidly absorbed after oral administration, with measurable blood concentrations detectable within 5-10 minutes following doses at levels of 22.5 mg-90 mg. The maximum plasma concentration is reached within 1 hour. The effect of food on absorption has not been studied. Butamirate is hydrolyzed to 2-phenylbutyric acid and diethylaminoethoxyethanol. It is assumed that both metabolites have antitussive activity. 2-phenylbutyric acid undergoes further metabolism. All metabolites are excreted mainly through the kidneys. Indications for use For the symptomatic treatment of dry cough of various origins. Method of administration and doses For oral administration. Take before meals. Children from 2 months to 1 year – 10 drops 4 times a day; from 1 to 3 years – 15 drops 4 times a day; from 3 years and older – 25 drops 4 times a day. 1 ml of the drug = 22 drops. The maximum duration of treatment should not exceed 7 days (see information in the Precautions section). If the cough persists for more than 7 days, you should consult a doctor. The need for further use of the drug is determined by the doctor, taking into account the characteristics of the disease, the tolerability of the drug and the effect achieved. Use during pregnancy and lactation This medicinal product is not intended for use in adults. The use of the drug during pregnancy and breastfeeding is not recommended. Precautions In view of the inhibition of the cough reflex by butamirate, simultaneous use with expectorants should be avoided, since this can lead to mucus congestion in the airways, which increases the risk of developing bronchospasm and respiratory tract infection. Due to the presence of sorbitol in the composition, drops should not be taken by patients with rare hereditary disorders associated with fructose intolerance. Drops contain saccharin and sorbitol as sweeteners, so they can be prescribed to patients with diabetes. The maximum duration of treatment should not exceed 7 days. If symptoms persist for more than 7 days, or the condition worsens, you should consult a doctor. This medicinal product contains a small amount of ethanol (2.81 mg/ml), less than 100 mg per dose. Due to the content of ethyl alcohol, the drug should be used with caution in children, patients with liver disease, alcoholism, epilepsy, traumatic brain injury and other brain diseases with a decrease in the seizure threshold. Use in patients with impaired renal and / or liver function No special studies have been conducted. Patients with impaired renal and / or liver function may be at greater risk of side effects due to the accumulation of butamirate or its metabolites. Interaction with other drugs Due to the proposed mechanism of action on the central nervous system, a possible increase in the effect of drugs that depress the central nervous system, including ethanol-containing drugs, cannot be ruled out. It is necessary to avoid the simultaneous use of other drugs containing ethyl alcohol. Concomitant use with expectorants should be avoided (see information in the Precautions section). Contraindications Hypersensitivity to the components of the drug, children under 2 months of age. Composition Active ingredient: butamirate citrate 5 mg/ml. Excipients: sorbitol, glycerin, sodium saccharin, vanillin, benzoic acid (E 210), ethanol 96%, sodium hydroxide, purified water. The medicinal product contains 0.30% ethanol. Overdose Overdose may cause the following symptoms: drowsiness, nausea, dizziness, vomiting, diarrhea, hypotension. Treatment: activated charcoal, saline laxatives, if necessary, maintaining the functions of the cardiovascular and respiratory systems. Side effects When using the drug, the following side effects are possible: From the side of the nervous system: Infrequently (from 0.1 to 1.0%): dizziness, drowsiness From the gastrointestinal tract: Infrequently (from 0.1 to 1.0%) : nausea, diarrhea Skin side effects: Rarely (from 0.01 to 0.1%): rash, urticaria.
