Transparent or opalescent solution from colorless to yellowish color with the smell of menthol and eucalyptus. Sympathomimetics. ATC CODE: R01AA07 Pharmacodynamics Xylometazoline is a sympathomimetic substance that affects the a-adrenergic receptors of the nasal mucosa (alpha-adrenergic agonist). When administered intranasally, the active substance has a vasoconstrictor effect on the vessels of the nasal cavity, helping to reduce swelling of the nasal mucosa and nasopharynx. Reducing swelling of the nasal mucosa contributes to the restoration of nasal breathing. The therapeutic effect after using the drug “Rinol with menthol and eucalyptus” occurs in a few minutes and lasts up to 12 hours. In therapeutic concentrations, it does not irritate the mucous membrane, does not cause hyperemia. The menthol and eucalyptus oil contained in the preparation have a cooling effect on the nasal mucosa. Pharmacokinetics When applied topically, it is practically not absorbed. Plasma concentrations are so low that they cannot be determined by analytical methods. Indications for use – various types of rhinitis (rhinitis during acute respiratory diseases, allergic and other types of rhinitis); – to facilitate the outflow of secretion in case of inflammation of the paranasal sinuses; – to reduce swelling of the nasopharyngeal mucosa with otitis media; – to facilitate rhinoscopy (diagnostic examination of the nasal cavity). Method of administration and doses For adults and children over 12 years old: 1 injection into both nostrils no more than 3 times a day. Repeat if necessary. It is recommended to use the last time before going to bed. Before use, clean the nasal passages. Do not use the drug for more than 7 days in a row (see information in the “Precautions” section). Before use, thoroughly clean the nasal passages. Remove the protective cap. Before the first use, press the sprayer several times until the spray begins to spray. With subsequent use, the spray will already be ready for use. If, after repeatedly pressing the sprayer, the spray does not come out completely, for example, after a temporary cessation of the use of the spray, press the sprayer again 4 times. Place the tip in the nasal passage, sharply press the sprayer 1 time, and at the same time inhale shallowly – this will ensure optimal distribution of the spray in the nasal cavity. After use, the protective cap must be put back on. Use in elderly patients Dose adjustment is not required. Recommended doses should not be exceeded. Since the systemic exposure of xylometazoline is very low, the likelihood of an effect on fertility is extremely small. Influence on working with mechanisms and ability to drive vehicles With long-term use and high doses of xylometazoline, possible systemic effects with cardiovascular effects cannot be ruled out. have a heart condition (such as long QT syndrome). Patients with long QT syndrome receiving xylometazoline may be at an increased risk of developing serious ventricular arrhythmias. The drug Rinol with menthol and eucalyptus should be taken with caution in patients: with arterial hypertension, diseases of the cardiovascular system with hyperthyroidism, diabetes mellitus, pheochromocytoma with prostatic hypertrophy receiving or treated with monoamine oxidase inhibitors within the last 14 days (see “Interactions with other drugs”) with hypersensitivity to sympathomimetics, which is manifested by symptoms such as insomnia, dizziness, etc. It is not recommended to use the drug Rinol with menthol and eucalyptus continuously for more than 1 week, as well as other topical vasoconstrictors, since it is possible to develop drug-induced rhinitis with swelling of the nasal mucosa, the symptoms of which are similar to the common cold. The recommended dose should not be exceeded, especially in elderly patients. Rinol with menthol and eucalyptus dosed nasal spray 0.1% with menthol and eucalyptus should not be used in children under 12 years of age. Benzalkonium chloride, which is part of the drug, can cause irritation and swelling of the nasal mucosa, especially with prolonged use. Interaction with other drugs Monoamine oxidase inhibitors. Xylometazoline can enhance the effect of monoamine oxidase inhibitors and cause a hypertensive crisis. Xylometazoline is not recommended for patients receiving or receiving treatment with monoamine oxidase inhibitors in the past 14 days (see Precautions). Tri- or tetracyclic antidepressants. Simultaneous use of tri- or tetracyclic antidepressants with sympathomimetic agents is not recommended, as it may increase the sympathomimetic effect of xylometazoline. Contraindications Hypersensitivity to the components of the drug, angle-closure glaucoma, atrophic rhinitis, dry rhinitis, transsphenoidal removal of the pituitary gland (or transnasal or transoral surgery with exposure of the dura mater). Composition Active substances: 1 ml of the drug contains 1.0 mg of xylometazoline hydrochloride as an active substance. Excipients: eucalyptus oil, levomenthol, benzalkonium chloride, sodium dihydrogen phosphate dihydrate, disodium phosphate dodecahydrate, disodium edetate, sodium chloride, concentrated hydrochloric acid, macrogolglycerin hydroxystearate, purified water. Overdose Excessive topical use of xylometazoline or accidental ingestion may cause sympatholytic effects, including CNS depression (eg, drowsiness, coma), hypertension or hypotension, tachycardia, and bradycardia. Gastrointestinal symptoms such as nausea and vomiting may also occur. Other symptoms are pallor, excessive sweating, hypothermia, pupillary constriction, respiratory depression, ataxia and anxiety. Severe symptoms have not been observed after accidental overuse. In one newborn (age 2 weeks), the use of 1 drop of a 0.1% solution in each nasal passage resulted in coma. In young children, the use of the drug up to 0.5 mg / kg of body weight did not cause clinically relevant symptoms. Due to the lack of data on the use of more than 0.5 mg / kg body weight, severe symptoms cannot be excluded. In such cases, medical supervision after consultation with an experienced clinician or poison information center and administration of a single dose of activated charcoal are indicated. There is no specific antidote. Bradycardia can be treated with atropine. Side effects Information on adverse reactions is presented according to system organ classes and frequency of occurrence in the following categories: very often (> 1/10), often (> 1/100 to <1/10), infrequently (> 1/1000 to < 1/100), rare (>1/10000 to <1/1000), very rare (<1/10000).
