Description:
Homogeneous suspension from white to grayish-white color. Pharmacological action It has anti-inflammatory, anti-allergic, anti-edematous action. The anti-inflammatory effect is realized as a result of the interaction of the drug with GCS receptors. Suppresses the proliferation of mast cells, eosinophils, lymphocytes, macrophages, neutrophils. Fluticasone propionate reduces the production of inflammatory mediators and other biologically active substances (histamine, PG, leukotrienes, cytokines) during the early and late phases of an allergic reaction. Restores the patient’s response to bronchodilators, allowing to reduce the frequency of their use. Reduces sneezing, nasal itching, runny nose, nasal congestion, sinus discomfort and pressure around the nose and eyes. It also relieves eye symptoms associated with allergic rhinitis. Indications for use Prevention and treatment of seasonal allergic rhinitis (including hay fever) and perennial rhinitis. Contraindications Hypersensitivity to fluticasone propionate or other ingredients of the drug. Pregnancy and lactation. Chronic atrophic rhinitis, children under 4 years of age. Route of administration and dosesNazofan is intended for intranasal administration only. Before the first use of Nasofan, the vial must be brought to readiness by pressing and releasing the pump 6 times. Do not use the drug 3 months after the first use. Adults and children over 12 years of age The recommended dose is 2 irrigations in each nostril once a day (200 mcg), preferably in the morning. In some cases, the dose may be 2 irrigations in each nostril 2 times a day (400 mcg). After achieving a therapeutic effect, it is possible to use a maintenance dose – 1 irrigation in each nostril 1 time per day (100 mcg). If the symptoms of the disease reappear, the dose may be increased accordingly. The maximum daily dose should not exceed 4 irrigations in each nostril (400 mcg). The minimum dose sufficient to obtain a therapeutic effect should be used. Elderly Patients Usual adult dosage is used. Children aged 4-12 years The recommended dose is 1 irrigation in each nostril 1 time per day (100 mcg), preferably in the morning. In some cases, the dose may be 1 irrigation in each nostril 2 times a day (200 mcg). The maximum daily dose should not exceed 2 irrigations in each nostril (200 mcg). The lowest dose sufficient to effectively control symptoms should be used. Children under 4 years of age: Since there are no sufficient data on the safety and efficacy of Nazofan in children under 4 years of age, it is not recommended to use the drug in patients of this age group. To achieve a greater therapeutic effect, the drug must be used regularly. The lack of an immediate effect is explained by the fact that maximum relief can be achieved no earlier than 3-4 days from the start of treatment. Side effects Probable side reactions and their frequency are given in the table. Inhalation or oral use of corticosteroids in high doses for a long time can cause hypothalamic-pituitary-adrenal depression. Interaction with other drugs Experimental studies did not reveal any significant effect of fluticasone on the pharmacokinetics of terfenadine and erythromycin. The effect of terfenadine on the pharmacokinetics of fluticasone was also insignificant. Fluticasone propionate is used with caution in combination with potent inhibitors of the cytochrome P450 ZA4 system (for example, protease inhibitors such as ritonavir). The simultaneous use of fluticasone propionate and 100 mg of ritonavir (a potent inhibitor of the cytochrome P450 ZA4 system) 2 times a day increased the concentration of fluticasone propionate in the blood plasma hundreds of times, which was the cause of a marked decrease in the concentration of cortisol in the blood serum. Cushing's syndrome and adrenal suppression have been reported. The use of such a combination should be avoided, unless the expected benefit outweighs the potential risk of systemic side effects of corticosteroids. Other inhibitors of the cytochrome P450 3A4 system cause a very slight (erythromycin) and a slight (ketoconazole) increase in the systemic effect of fluticasone propionate without a noticeable decrease in the concentration of cortisol in the blood serum. Do not take any other medicines (including those available without a prescription) while you are being treated with Nazofan without first talking to your doctor.
