Description:
Pills of non-uniform pink color, round, flat on both sides, with cut edges; with a score on one side, allowing the tablet to be divided in half. The main active ingredient Molsidomin Release form Tablets Dosage 4 mg Special instructions The drug contains lactose and sucrose. If the patient has previously established intolerance to any sugars, then before taking the drug, he should inform the doctor about this. Pharmacological action Molsidomin dilates arterial vessels and reduces spasm of the coronary arteries. Reduces the heart’s need for oxygen and facilitates its work, which increases exercise tolerance and reduces the frequency of angina attacks. Indications for use Prevention of angina attacks. Chronic heart failure (as part of combination therapy with cardiac glycosides, diuretics). Method of administration and dosage The dose of the drug is selected by the doctor individually and depends on the severity of the disease, the patient’s condition and concomitant therapy. Tablets should be taken at regular intervals with a sufficient amount of liquid (about 1/2 cup). The drug can be taken before, during or after meals. Usually Dilasid is prescribed 1-2 mg 2-3 times a day. If necessary, the dose can be increased to 4 mg 3-4 times a day (up to 12-16 mg of molsidomine per day). If it becomes necessary to increase the dose, this should be done gradually to avoid the development of side effects such as headache. Features of dosing in patients with pathology of the liver and (or) kidneys and elderly patients are presented in the “Special Instructions” section. Missing a dose of a drug Do not take a double dose to make up for the missed one. Use during pregnancy and lactation If the patient is pregnant, or breastfeeding, or suspects that she may be pregnant, or when planning a pregnancy, she should consult a doctor or pharmacist before using this drug. The use of molsidomine during pregnancy and during breastfeeding is contraindicated. If it becomes necessary to use the drug Dilasid in women who are breastfeeding, it is necessary to stop breastfeeding. The ability to influence the reaction rate when driving vehicles or working with other mechanisms During the period of treatment with Dilasid (especially at the beginning), it is necessary to refrain from driving vehicles and engaging in potentially hazardous activities that require increased concentration and speed of psychomotor reactions. Precautions The drug is prescribed with caution: patients with cerebral circulation disorders, with increased intracranial pressure, with a tendency to arterial hypotension, elderly patients, after myocardial infarction, patients with glaucoma (especially angle-closure), with hypertrophic obstructive cardiomyopathy, constrictive pericarditis, cardiac tamponade, aortic and mitral stenoses. Molsidomine is not applicable for the relief of angina attacks! In the acute phase of myocardial infarction, molsidomine can be used only after stabilization of blood circulation. The drug can be combined with other antianginal agents (for example, added to two- or three-component treatment – nitrates, slow calcium channel blockers and beta-adrenergic receptor blockers). During treatment, alcohol should be excluded. The simultaneous use of molsidomine and PDE-5 inhibitors (sildenafil, tadalafil, vardenafil) is contraindicated due to the possibility of a sharp decrease in blood pressure with fainting and even collapse. Before prescribing molsidomine, the doctor should inform the patient about the impossibility of simultaneous treatment with molsidomine and PDE-5 inhibitors. If necessary, you can take molsidomine no earlier than 24 hours after taking PDE-5 inhibitors. Patients with impaired liver and kidney function In patients with impaired liver and kidney function, the doctor may modify the dosage depending on the patient’s condition. In patients with hepatic insufficiency, the doctor may prescribe treatment from the lowest dose and gradually increase it until the desired therapeutic effect is achieved. Elderly patients Elderly patients usually do not require dose adjustment. However, in elderly patients with reduced circulating blood volume or taking other drugs that dilate blood vessels, it is recommended to use the drug with caution due to the risk of hypotension. Interactions with other drugs Tell the doctor or pharmacist about all medicines that the patient is taking or has taken recently, as well as those that the patient plans to take. Molsidomin enhances the action of vasodilators (drugs that dilate blood vessels), which can lead to a sharp drop in blood pressure. With simultaneous use with acetylsalicylic acid, its antiplatelet activity is enhanced. With the simultaneous use of molsidomine and iloprost, a pronounced inhibition of platelet aggregation may occur. In patients who simultaneously use iloprost and molsidomine, tests should be performed to evaluate the blood picture and platelet aggregation. During the use of molsidomine, the use of PDE-5 inhibitors (sildenafil, tadalafil, vardenafil) is contraindicated, as a sudden pronounced drop in blood pressure with dangerous consequences may develop. There is an increased risk of developing arterial hypotension while taking molsidomine and sapropterin. Alcohol enhances the action of molsidomine. Contraindications Hypersensitivity to molsidomine or any of the excipients of the drug. Shock. Acute anginal attack, unstable angina pectoris, acute myocardial infarction. Vascular collapse or severe arterial hypotension (systolic blood pressure less than 100 mm Hg). Decreased central venous pressure. Pregnancy. breastfeeding period. Simultaneous use of phosphodiesterase type 5 inhibitors (PDE-5) – sildenafil, tadalafil, vardenafil – due to the risk of arterial hypotension. Toxic pulmonary edema. Age up to 18 years. Composition 1 tablet contains: Active substance: molsidomine 4.0 mg. Excipients: lactose monohydrate, sucrose, potato starch, aluminum lacquer based on crimson dye (Ponso 4R) (E 124), povidone K-25, magnesium stearate. Overdose Symptoms of overdose are: severe headaches, marked decrease in blood pressure, tachycardia. In case of taking the drug in a dose greater than recommended, you should immediately consult a doctor. The doctor may decide to do a gastric lavage. If necessary, may prescribe symptomatic treatment. Cardiovascular disorders: often - a pronounced decrease in blood pressure, sometimes up to collapse; rarely - tachycardia, orthostatic hypotension, redness of the skin of the face. Immune system disorders: rarely - allergic reactions (skin rash, itching, bronchospasm); with an unknown frequency - the development of anaphylactic shock. Blood and lymphatic system disorders: thrombocytopenia.
