Thiocetam tabl.p/o 400mg/100mg in a cell.pack No. 10×6
Description:
Tablets, film-coated yellow, oval in shape with a biconvex surface, with a risk on one side of the tablet. On the fault, a white core is visible. The main active ingredient Piracetam Release form Film-coated tablets. Dosage 400mg/100mg in a cell. pack No. 10×6 Pharmacodynamics The drug has antioxidant, membrane stabilizing and nootropic properties. The drug improves the integrative and cognitive activity of the brain, helps to eliminate the symptoms of amnesia, increases the performance of short-term and long-term memory. The pharmacological effect of the drug is due to the mutually potentiating action of thiazoic acid and piracetam. The drug is able to accelerate the oxidation of glucose in the reactions of aerobic and anaerobic oxidation, influence bioenergy processes, increase the level of ATP. The drug inhibits the formation of reactive oxygen species, reactivates the antioxidant system of enzymes, especially superoxide dismutase, inhibits free radical processes in brain tissues during ischemia, improves the rheological properties of blood by activating the fibrinolytic system. PharmacokineticsPiracetam is well absorbed when used internally, penetrates into various organs and tissues, including brain tissue. The maximum plasma concentration is reached after 1.5 hours, bioavailability is close to 100%. The volume of distribution of piracetam is 0.7 l / kg, the half-life is 5 hours. The drug crosses the placental barrier. Piracetam is practically not metabolized in the body and is excreted in the urine. Thiazoic acid is metabolized in the liver and excreted in the urine. Indications for use Symptomatic treatment of memory disorders and intellectual disorders in the absence of a diagnosis of dementia (symptomatic treatment of chronic psychoorganic syndrome) in patients with chronic cerebrovascular disorders (dyscirculatory encephalopathy), diabetic encephalopathy, ischemic stroke or traumatic brain injury. Dosage and administration Dosage and duration of treatment is determined by the doctor in each individual case separately, depending on the nature and course of the disease. In chronic disorders of cerebral circulation and in the rehabilitation period after ischemic stroke, 1 tablet 3 times a day for 25-30 days. Tiocetam tablets are prescribed 30 minutes before meals. The course of treatment is from 2-3 weeks to 3-4 months. For the treatment of diabetic encephalopathy, 1 tablet is prescribed 3 times a day for 45 days. Patients with renal insufficiency require dose adjustment of the drug in accordance with the following scheme: Renal insufficiency Creatinine clearance (ml / min) Dose and frequency of use Norm > 80 Usual dose Mild degree 50-79 2/3 of the usual dose in 2-3 doses Moderate degree 30-49 1/3 usual dose in 2 doses Severe <30 1/6 usual dose, single dose End stage - Contraindicated In elderly patients, the dose is adjusted in the presence of renal insufficiency; with prolonged therapy, it is necessary to monitor the functional state of the kidneys. Patients with impaired liver function dose adjustment is not required. For patients with impaired renal and hepatic function, doses are adjusted depending on creatinine clearance (see above). Children. The drug is contraindicated for use in children under 18 years of age due to lack of sufficient data. Use during pregnancy and lactation The safety of the drug during pregnancy and lactation has not been established. Due to the lack of data, use during pregnancy and lactation is contraindicated. Precautions With caution, the drug is prescribed to patients with chronic renal failure. With caution, the drug should be used in elderly people with cardiovascular pathology due to the fact that the adverse reactions described above are more common in this group of patients. Allergic reactions are more common in people prone to allergies. Due to the effect of piracetam on platelet aggregation, caution should be exercised when prescribing the drug to patients with impaired hemostasis, surgical intervention (including dental), risk factors for bleeding (for example, peptic ulcer of the stomach and duodenum), previous hemorrhagic stroke or intracerebral hemorrhage, patients taking anticoagulants or antiplatelet agents, including low doses of acetylsalicylic acid. Elderly patients. With long-term therapy in elderly patients, regular monitoring of renal function is recommended, if necessary, adjust the dose depending on the results of the study of creatinine clearance. Due to the lactose content, the drug should not be administered to patients with galactose intolerance, lactase deficiency or glucose-galactose malabsorption. Application in children. The drug is contraindicated for use in children under 18 years of age due to lack of sufficient data. Interaction with other drugsThiocetam should not be administered with drugs that have an acidic pH. Due to the presence in the composition of piracetam, the following types of interaction are possible: Thyroid hormones. When used together with thyroid hormones (T3 + T4), increased irritability, disorientation and sleep disturbance are possible. Acenocoumarol. In patients with severe recurrent thrombosis, the use of high doses of piracetam (9.6 g/day) did not affect the dosage of acenocoumarol to achieve a prothrombin time (INR) of 2.5–3.5, but with its simultaneous use, there was a significant decrease in the level platelet aggregation, fibrinogen level, von Willebrand factors (coagulation activity (VIII: C); co-factor ristocetin (VIII: vW: Rco) and plasma protein (VIII: vW: Ag)), blood and plasma viscosities. Pharmacokinetic interactions. The likelihood of changes in the pharmacokinetics of piracetam under the influence of other drugs is low, since 90% of the drug is excreted unchanged in the urine. In experiments in vitro, piracetam does not inhibit cytochrome P450 isoforms CYP1A2, 2B6, 2C8, 2C9, 2C19, 2D6,2E1 and 4A9 / 11 at a concentration of 142,426,1422 μg / ml. For a concentration of 1422 μg / ml, minimal inhibition of CYP2A6 (21%) and 3A4 / 5 (11%) was observed. However, normal values of the inhibition constant (Ki) can probably be achieved at a higher concentration. Thus, the metabolic interaction of piracetam with other drugs is unlikely. Antiepileptic drugs. Taking piracetam at a dose of 20 g / day for 4 weeks in patients with epilepsy who received stable doses of antiepileptic drugs did not change the maximum serum concentration and AUC (area under the curve) of antiepileptic drugs (carbamazepine, phenytoin, phenobarbital and valproate). Simultaneous reception with enalapril, captopril increases the risk of adverse reactions from the cardiovascular system. Alcohol. Co-administration with alcohol did not affect the serum concentration of piracetam, and the concentration of ethanol in the blood serum did not change when using 1.6 g of piracetam. Contraindications - hypersensitivity to piracetam, thiazoic acid, other components of the drug, as well as to derivatives of pyrrolidone or thiazoic acid; - end-stage renal failure (creatinine clearance less than 20 ml / min); - Huntington's chorea; - hemorrhagic stroke; - period of pregnancy; - the period of breastfeeding. Active ingredients: 1 tablet contains piracetam - 400 mg; morpholinium salt of thiazoic acid - 100 mg, which is equivalent to 66.5 mg of thiazoic acid. excipients: potato starch; mannitol; powdered sugar; magnesium stearate; Povidone, Opadry II Yellow Coating Mix 33G22623 (Hypromelose; Lactose Monohydrate; Titanium Dioxide (E 171); Polyethylene Glycol (Macrogol) 3000; Triacetin; Quinoline Yellow (E 104); Sunset Yellow FCF (E 110); Iron Oxide yellow (E 172); indigo carmine (E 132)). Overdose If you deviate from the doses recommended by your doctor, you may experience or increase the side effects of the drug (excitation, sleep disturbance, dyspepsia). In such cases, stop the use of the drug and prescribe symptomatic treatment.
