Special instructions The drug is contraindicated in children and adolescents under 18 years of age. Indications for use As part of complex therapy in patients with grade I-II hemorrhoids in the acute phase. Dosage and administration Use the drug after hygiene procedures. Wash the skin around the anus with warm water or clean it with a moistened soft cloth, dry gently with toilet paper or a soft cloth. Before the introduction of the suppository, it is necessary to remove the protective plastic shell from it. The drug is injected into the rectum 1 suppository 2 times a day until the disappearance of acute clinical symptoms. After improving the clinical picture, they switch to the introduction of 1 suppository 1 time per day. The duration of the course of treatment is determined by the attending physician. Interaction with other drugs No interaction studies have been conducted with other medicinal products. Cholinesterase inhibitors inhibit the metabolism of benzocaine. Benzocaine reduces the antimicrobial activity of sulfonamides. Cetylpyridinium is incompatible with soaps and other anionic surfactants. Papaverine hydrochloride, when used simultaneously, potentiates the effect of alcohol, weakens the hypotensive effect of methyldopa, can reduce the anti-Parkinsonian effect of levodopa, enhance the effect of calcium channel blockers and drugs that depress the central nervous system, and shows synergism in combination with morphine. Nicotine is able to reduce or completely eliminate the vasodilating effect of papaverine hydrochloride. When combined with antiarrhythmic and antihypertensive drugs, it is possible to increase the hypotensive effect. Metamizole (analgin), diphenhydramine (diphenhydramine), diclofenac potentiates the antispasmodic effect of papaverine hydrochloride. Do not co-administer with O-adrenergics (risk of arrhythmias). The effects of papaverine are potentiated by anesthetic, analgesic, sedative and tranquilizing substances. When combined with tricyclic antidepressants, procainamide, reserpine, quinidine, it is possible to increase the hypotensive effect of papaverine, with cardiac glycosides, there is a pronounced increase in myocardial contractile function due to a decrease in total peripheral vascular resistance. There is evidence of the development of hepatitis when combined with furadonin. The risk of developing methemoglobinemia is higher in young children, the elderly, patients with hereditary disorders (deficiency of the enzymes glucose-6-phosphodiesterase, pyruvate kinase, NADH-methemoglobin reductase), heart disease, respiratory diseases (bronchial asthma, bronchitis, pulmonary emphysema), in case of smoking. Use with caution in elderly and debilitated patients, in patients with traumatic brain injury, adrenal insufficiency, hypothyroidism, prostatic hyperplasia, supraventricular tachycardia. With prolonged use of the drug, monitoring of blood counts is necessary. During the period of treatment, alcohol should be excluded. Overdose In case of a significant excess of the recommended doses, there may be a tendency to hypercoagulability, impaired vision (diplopia), weakness, drowsiness, dizziness, tachycardia, hypotension, heart rhythm disturbance, development of atrioventricular blockade, respiratory paralysis, intestinal atony, difficulty urinating. In the case of systemic absorption of benzocaine in case of overdose, disorders may develop: drowsiness, anxiety, agitation, in severe cases, convulsions. Treatment is symptomatic. In case of accidental ingestion of the drug, gastric lavage and enterosorbents are indicated. With an overdose of drugs containing benzocaine, the risk of developing methemoglobinemia (cyanosis of the skin, lips, nail plates, headache, dizziness, tachycardia, general weakness, respiratory failure) increases, which can be fatal and requires emergency medical care.
