Description:
Lucetam 800mg tablets are a nootropic medication used for the symptomatic treatment of intellectual-mnestic disorders and the reduction of cortical myoclonus. Each pack contains 30 film-coated tablets, white or almost white, oval, biconvex, chamfered, scored on both sides. They are engraved with “E 242” on one side and are odorless.

Pharmacological Action:
Lucetam is a nootropic drug that acts on the central nervous system in multiple ways:
- Modifies neurotransmission in the brain.
- Enhances neuronal plasticity.

- Does not cause vasodilation.
Lucetam improves concentration, cognitive functions, and EEG activity in patients with cerebral dysfunction. It promotes recovery from cognitive impairments caused by hypoxia, intoxication, or electroconvulsive therapy. It also reduces the duration of vestibular neuronitis.
Piracetam inhibits platelet aggregation and improves erythrocyte deformability and filtration capacity in cases of pathological rigidity.
Pharmacokinetics:
- Absorption: Rapid and near-complete absorption from the gastrointestinal tract. Bioavailability is approximately 100%.
- Distribution and Metabolism: Distributed widely throughout the body, with selective accumulation in the cerebral cortex, cerebellum, and basal nuclei. Does not bind to plasma proteins. Crosses the blood-brain barrier and placental barrier. Not metabolized in the body.
- Elimination: Half-life in blood plasma is 4-5 hours, and in cerebrospinal fluid is 8.5 hours. Excreted unchanged by the kidneys.
Indications for Use:
- Symptomatic treatment of intellectual-mnestic disorders in the absence of dementia diagnosis.
- Reduction of cortical myoclonus, as monotherapy or part of a combination regimen.
Dosage and Administration:
The daily dose is divided into 2-4 doses, with the last dose taken no later than 5 pm to avoid sleep disturbances.
- Intellectual-mnestic disorders: 2.4-4.8g/day, divided into 2-3 doses.
- Cortical myoclonus: Starting dose of 7.2g/day, increased by 4.8g/day every 3-4 days until a maximum of 24g/day is reached. Treatment is continued throughout the course of the disease.
To prevent worsening of the condition, the dose should be gradually reduced by 1.2g/day every 2 days. Treatment is stopped if there is no effect or only minimal therapeutic effect.
Renal Insufficiency:
Dosage adjustments are necessary based on creatinine clearance. Treatment is prescribed depending on the severity of renal failure.
Elderly Patients:
Dosage adjustment is necessary in the presence of renal insufficiency. Regular monitoring of kidney function is recommended.
Pregnancy and Lactation:
Lucetam should not be used during pregnancy. Piracetam crosses the placental barrier and is excreted in breast milk. Breastfeeding should be discontinued if Lucetam is necessary during lactation.
Precautions:
- Dosage correction is not required for impaired liver function.
- Dosage adjustment is required for impaired renal function.
- Dose adjustment is necessary in elderly patients with renal failure.
- Long-term therapy requires monitoring of kidney function.
Drug Interactions:
- Increased irritability, disorientation, and sleep disturbances are possible with simultaneous use of thyroid extract (T3 + T4).
- No interactions have been reported with clonazepam, phenytoin, phenobarbital, or valproic acid.
- High doses of Lucetam (9.6g/day) can increase the effectiveness of acenocoumarol in patients with venous thrombosis.
- The possibility of altering Lucetam’s pharmacodynamics due to other drugs is low, as 90% of the drug is excreted unchanged in the urine.
- In vitro studies show minimal interaction with CYP isoenzymes.
- Co-administration with ethanol does not affect serum Lucetam concentration, and Lucetam does not affect blood ethanol concentration.
Contraindications:
- Huntington’s chorea
- Hemorrhagic stroke
- Severe chronic renal failure (creatinine clearance less than 20ml/min)
- Pregnancy
- Lactation period
- Hypersensitivity to piracetam or pyrrolidone derivatives
With caution:
- Impaired hemostasis
- Extensive surgical interventions
- Severe bleeding
- Chronic renal failure (creatinine clearance 20-80ml/min)

Overdose:
Lucetam is non-toxic even at high doses. In rare cases, dyspeptic symptoms like diarrhea with blood and lower abdominal pain have been reported.
