The adult human body contains 2 to 3 g of zinc. The daily requirement is about 15 mg. 90% of the zinc in the systemic circulation is found in leukocytes and erythrocytes (mainly as part of the carbonic anhydrase enzyme), in the blood plasma – only 10% (0.1% of the total zinc contained in the body), mainly in connection with albumin, less with alpha-2 macroglobulin and transferrin. Zinc plays a significant role as a structural, catalytic, and regulatory component in enzymes such as alkaline phosphatase, various dehydrogenases, thiamidine kinases, carboxypeptidases, DNA-dependent RNA polymerases, and (along with copper) superoxide dismuth. In the pancreas, zinc is involved in the formation of insulin and glucagon aggregates. Zinc has a significant impact on the production of male and female sex hormones, affecting sexual function and reproduction. In the mechanisms of cellular and humoral immune response, zinc is involved in mitogenic stimulation of immune cells, affects the function of T-lymphocytes, macrophages and granulocytes. Zinc deficiency is especially noticeable in the implementation of its biochemical functions. In this case, rapidly growing tissues are most susceptible (for example, spermatozoa, embryonic and fetal cells, immunocompetent cells, connective tissue cells involved in the regeneration of damage). Zinc is essential for DNA and RNA dependent protein synthesis. Pharmacokinetics From the intestines, 10 to 40% of the zinc supplied with food is absorbed. Absorption is carried out mainly in the duodenum and upper small intestine. Zinc resorption is very variable and depends on many factors. Protein-rich foods increase zinc absorption, while legumes and grains, due to the presence of phytic acid in them, form poorly soluble and poorly absorbed complexes with zinc. Cadmium, copper, calcium, and iron can also interfere with zinc absorption. Zinc is excreted mainly with feces, renal excretion ranges from 0.3 to 0.6 mg per day. The concentration of zinc in tissues is variable. With the help of a radioactive label, it was found that 40% of zinc enters the liver, decreasing to 25% within 5 days. A number of factors, especially hormones, affect the amount of zinc in the liver. In the liver, as in other tissues, zinc binds to metallothionein. The highest concentrations of zinc are found in bones, skin, prostate, retina, and spermatozoa. Indications for use It is used to treat conditions caused by zinc deficiency, which cannot be eliminated by dietary modification. Method of administration and doses For adults and adolescents (12-17 years old) the recommended dose is ½ – 1 tablet per day. Tablets are taken orally before meals (approximately 60 minutes before meals), with a sufficient amount of liquid. The tablets should not be taken with food. Exception: At the beginning of treatment, zinc preparations can cause abdominal pain, nausea, dyspepsia and diarrhea. These complaints are more common when zinc supplements are taken on an empty stomach. In this case, the drug should be taken exclusively with meals. The duration of administration is determined by the therapeutic effect. Children: There is no experience of use in children. Elderly: There are no special restrictions on use in the elderly. ContraindicationsIncreased individual sensitivity to the components of the drug. Precautions For long-term use, laboratory monitoring of the level of zinc and copper in the blood is recommended. Interaction with other medicinal products Chelating agents such as D-penicillamine, dimercaptopropanesulfonic, dimercaptosuccinic or ethylenediaminetetraacetic acids may reduce zinc absorption or increase its excretion. With the simultaneous intake of zinc and phosphates, iron salts, copper and calcium, the absorption of zinc may decrease. Zinc can disrupt the intake of copper in the body. High doses of zinc may reduce iron absorption and storage. Zinc reduces the absorption of tetracyclines, ofloxacin, and other fluoroquinolones (eg, norfloxacin, ciprofloxacin). In this regard, a time interval of at least three hours must be observed between taking zinc and these medicines. Foods high in phytin (eg, grains, legumes, nuts) reduce zinc absorption. There is evidence that coffee also reduces zinc resorption. Pregnancy and lactation Due to insufficient experience with human use, zinc preparations during pregnancy and lactation should only be used in cases of clinically proven zinc deficiency. However, when using therapeutic doses of Zincorot® 25, there are no restrictions on its use during pregnancy and lactation. Before using the drug during pregnancy or lactation, you should consult your doctor. Influence on the ability to drive vehicles and work with mechanisms There are no data on the effect of the drug on the ability to drive vehicles or work with potentially dangerous mechanisms. Side effectsThe following scale is used to assess the frequency of adverse reactions: very often (> 1/10), often (> 1/100, <1/10), sometimes (> 1/1000, <1/100), rarely (> 1 /10,000, <1/1000), very rare (<1/10000), including isolated reports, not known (frequency not estimated based on available data). Very rarely, taking the drug Zinkorot® 25 can cause complaints from the gastrointestinal tract in the form of abdominal pain, nausea, vomiting, diarrhea and symptoms of stomach irritation. These symptoms appear, as a rule, at the beginning of treatment and are observed more often when taken on an empty stomach than with food. In such cases, the drug should be taken as an exception during meals. Long-term zinc intake can lead to copper deficiency. Overdose Only when taken in a dose many times higher than the therapeutic one, signs of an overdose may appear, manifested by a metallic taste in the mouth, headaches and weakness, which quickly pass after the drug is discontinued.
