Dosage Adults and children over the age of 7 years are prescribed 1 tablet 1 time / day. Pikovit® Forte is not recommended for children under the age of 7 years. Children and adolescents under 14 years of age should only take this medicine on the advice of a doctor or pharmacist. Talk to your doctor or pharmacist before taking Pikovit® Forte. If you have diabetes, you should know that one tablet of Pikovit® Forte contains about 0.8 g of maltitol. Due to slow hydrolysis and gastrointestinal absorption, maltitol has little effect on blood sugar levels. If you have phenylketonuria, do not take this medicine because it contains aspartame, which contains phenylalanine. Talk to your doctor or pharmacist before taking other vitamin, vitamin/mineral and/or mineral supplements together. High doses of certain components, especially vitamin A and vitamin D, can be hazardous to health. It is possible that urine stains yellow, which is a completely harmless factor and is explained by the presence of riboflavin in the preparation. Pharmacodynamics Multivitamins. Pikovit forte tablets contain the 11 most important vitamins in amounts corresponding to the recommended daily dose for children over the age of 7 years. B vitamins (B1, B2, B6, B12, pantothenic acid and nicotinamide) are involved in the metabolism of carbohydrates, proteins and fats. Folic acid (vitamin Bc) is necessary for the normal synthesis, regeneration and functioning of blood cells. Retinol (vitamin A) is necessary for the development of epithelial cells and the synthesis of visual pigment. Colecalciferol (vitamin D) regulates calcium intake and promotes proper mineralization of bones and teeth. Ascorbic acid (vitamin C) promotes the absorption of iron and is involved in many redox processes in the body. Tocopherol (vitamin E) is a physiological antioxidant that protects cell membranes and supports cell function. Pikovit forte does not contain sugar. Pharmacokinetics Water-soluble vitamins (groups B, C) are well absorbed in amounts corresponding to the daily needs of the body. Amounts exceeding the tissue saturation level are excreted in the urine, sometimes in the feces. The regular use of these vitamins is important to maintain the necessary concentrations in the tissues, since they accumulate in the body in limited quantities. After oral administration, fat-soluble vitamins A and D are well absorbed in the small intestine in the presence of fat. Absorption of vitamin E is rather weak (25-85% of the dose). Indications for use caused by insufficient intake or increased need for vitamins, for example, in the absence of appetite, increased physical and psychological stress, with irregular or inadequate nutrition, or after antibiotic therapy. Pikovit® Forte does not replace a balanced and varied diet. Therefore, it is important to ensure that your child’s diet is balanced and varied, even while taking the drug. How to use Pikovit® Forte tablets should be kept in the mouth until completely resorbed. Interactions with other drugs Tell your doctor if you are taking, have recently taken or may start taking any other medicines. Due to the possibility of developing toxic reactions, Pikovit® Forte should not be used in conjunction with other preparations containing vitamin A or oral retinoids. Retinol may reduce the effect of glucocorticoids. Pikovit® Forte should not be taken simultaneously with cholestyramine because it interferes with the absorption of retinol. During sulfonamide therapy, large amounts of vitamin C should be avoided to prevent crystalluria. Vitamin C increases iron absorption, but slows down the absorption of beta-blockers and indirect anticoagulants. The simultaneous use of vitamin C and deferoxamine increases the tissue toxicity of iron, especially in the heart muscle, which can lead to decompensation of the circulatory system. Vitamin C can only be used 2 hours after deferoxamine injection. Calcium chloride, salicylates, corticosteroids with prolonged use reduce the reserves of ascorbic acid in the body. The absorption of ascorbic acid is reduced with the simultaneous use of fruit or vegetable juices, alkaline drinking. Vitamin B6 weakens the effect of levodopa if the patient takes only levodopa, prevents or reduces the toxic manifestations that are observed with isoniazid and other anti-tuberculosis drugs. Thiamine can weaken the curare-like effect. PAS, cimetidine, potassium preparations, alcohol reduce the absorption of vitamin B12. Riboflavin is incompatible with streptomycin and reduces the effectiveness of antibacterial drugs (oxytetracycline, doxycycline, erythrocycline, tetracycline and lincomycin). It should be taken at least 3 hours before or 3 hours after a dose of antibiotic. The tricyclic antidepressants imipramine and amitriptyline inhibit riboflavin metabolism, especially in cardiac tissues. With simultaneous use with quinine, hemorrhages may increase. With the simultaneous use of vitamin D3 and diuretics of the thiazide group, the risk of hypercalcemia increases. Ion exchange resins such as cholestyramine and laxatives (paraffin oil) can reduce the absorption of vitamin D3. Since the concomitant oral use of drugs containing calcium and vitamin D3 enhances the effect of cardiac glycosides, medical supervision and ECG monitoring during their simultaneous use is necessary. In folate-deficient patients, the use of folic acid may reduce plasma levels of phenobarbital and cause an epileptic seizure. Ethanol, cycloserine, glutethimide, and methotrexate may interfere with folate metabolism. The absorption of folic acid decreases with simultaneous use with analgesics, anticonvulsants, antacids, sulfonamides, antibiotics, cytostatics. Cases of reduced or altered absorption may occur with the simultaneous use of cholestyramine and folic acid. Therefore, the drug should be taken 1 hour before or 4-6 hours after taking cholestyramine. Do not use together with mineral acids, alkaline substances, reducing agents, as folic acid is inactivated. The drug is not intended for children under the age of 7 years. Composition of 11 essential vitamins – antioxidants A, C, E and group B, Nicotinamide, Folic and Pantothenic acid. Overdose When taking the recommended doses, the likelihood of overdose is minimal. In case of an overdose, the likelihood of these adverse reactions increases, bloating (flatulence) may occur. Prolonged use of high doses can cause hypervitaminosis A and D3, although overdose of the drug is unlikely. With an overdose of vitamin D3, weakness, anorexia, nausea, vomiting, diarrhea, weight loss, fever, convulsions, disorders of the cardiovascular system and kidneys occur. Perhaps the development of hypercalcemia due to hypersensitivity to vitamin D3. Symptoms of hypercalcemia are anorexia, polyuria, nausea, vomiting, general weakness, headache, apathy, thirst. Overdose can lead to changes in the renal excretion of ascorbic and uric acids during acetylation of urine with the risk of formation of oxalate stones. The use of large doses of ascorbic acid can lead to heartburn. If the recommended doses are significantly exceeded (more than 1 g per day, which corresponds to 17 tablets of the drug), renal failure, sleep disturbances, a feeling of heat, increased fatigue, impaired zinc and copper metabolism, irritability, erythrocytopenia, neutrophilic leukocytosis may occur. Very high doses of folic acid can increase CNS excitability and cause seizures. The sweeteners mannitol and maltitol are poorly absorbed in the digestive tract. At higher doses, they cause diarrhea and bloating. The laxative effect occurs when consuming more than 60 g of maltitol (more than 70 Pikovit® Forte tablets) or more than 10 g of mannitol (more than 77 Pikovit® Forte tablets) daily. If signs of overdose appear, the drug should be discontinued. If possible, you should: induce vomiting and hydrate the body. If hypercalcemia is detected, a diet with a limited amount of calcium and vitamin D3 should be followed. Further treatment is symptomatic.
