Dosing and Administration The usual dose is 1-2 capsules 1-3 times a day for 30 days. The doctor determines the individual dose and duration of treatment. Use during pregnancy and lactation There are no data on the negative effect of Neuromed on the fetus and child during pregnancy and lactation. Vitamins penetrate the placental barrier and are excreted from the body with mother’s milk. Due to the lack of sufficient information, the use of the drug is possible only in case of absolute necessity, in recommended doses, as prescribed and under the supervision of a physician, after assessing the benefit / risk ratio, if the benefit outweighs the risk. It should be taken into account that at high doses, pyridoxine may interfere with prolactin secretion and should be used with caution in nursing mothers. The use of large amounts during pregnancy can lead to pyridoxine dependence syndrome in newborns. Precautions With extreme caution and in minimal doses, Neuromed should be used in patients with severe and acute forms of decompensated heart failure and angina pectoris. With prolonged use (more than 6-12 months) of vitamin B6 in a daily dose of 50 mg, peripheral sensory neuropathy may occur. Therefore, during long-term therapy, it is recommended to regularly monitor the patient’s condition. If signs of peripheral sensory neuropathy (paresthesia) appear, it is necessary to adjust the dose and, if necessary, stop taking the drug. When taking vitamin B12, the clinical picture, as well as laboratory parameters for funicular myelosis or pernicious anemia, may lose their specificity. This medicinal product contains lactose as an excipient and should not be taken by patients with lactase deficiency, galactosemia or glucose/galactose malabsorption syndrome. Capsules are not intended for use in children under 6 years of age, as this group of patients may have difficulty swallowing medicines in the form of capsules. If symptoms persist or worsen while taking the drug, you should consult a doctor. Interaction with other drugs When used together with ethanol-containing drugs, the absorption of thiamine is sharply reduced (blood levels may decrease by 30%). Antacids, tea reduce the absorption of vitamin B1. Thiamine is inactivated by 5-fluorouracil, which competitively inhibits thiamine phosphorylation. Vitamin B6 reduces the antiparkinsonian effect of levodopa. This vitamin enhances the peripheral decarboxylation of levodopa and thus reduces its activity in the treatment of Parkinson’s disease. High doses of vitamin B6 counteract the action of isoniazid and thiosemicarbazone, along with the elimination of sideroblastic anemia induced by these anti-tuberculosis drugs. Prolonged use of penicillamine can cause vitamin B6 deficiency. Hydralazine and cycloserine are also its antagonists, and their combined use with vitamin B6 reduces the undesirable neurological effects of these drugs. The amount of vitamin B6 decreases during simultaneous use with oral contraceptives. With long-term treatment with furosemide, an increase in renal excretion of thiamine is possible. Long-term treatment with anticonvulsants (phenobarbital, phenytoin, carbamazepine) in combination with Neuromed may lead to thiamine deficiency. Absorption of vitamin B12 is inhibited by colchicine, ethanol and neomycin. Oral biguanide antidiabetic drugs and para-aminosalicylic acid, as well as chloramphenicol and vitamin C, impair the absorption of this vitamin. Vitamin B12 is not compatible with salts of heavy metals. Vitamin B12 After high doses with parenteral use (in rare cases after oral use) – allergic reactions, skin eczema and mild acne. Treatment: should be supportive and symptomatic. Side effects Neuromed is generally well tolerated. In very rare cases, allergic reactions may occur – itching, urticaria, shortness of breath, Quincke’s edema, anaphylactic shock.
