Lidocaine is rapidly absorbed through mucous membranes (especially the tracheal mucosa), as well as through injured skin. This should be taken into account, especially when the drug is applied to large areas or used in children. Care should be taken when applying the drug to patients on the affected mucous membranes and / or infected areas. Caution should be exercised when using the drug in patients with epilepsy, as well as in bradycardia, conduction disturbances, impaired liver function, shock, especially if the absorption of a significant amount of the drug is expected based on the applied dose and the treated area. Debilitated patients, elderly patients, seriously ill patients and children need to use smaller doses of the drug according to their age and general condition. For children under the age of 2 years, it is advisable to apply Lidocaine-Health with a cotton swab soaked in the preparation. Pharmacological action Class IB antiarrhythmic agent, local anesthetic, acetanilide derivative. It has membrane stabilizing activity. Causes blockade of sodium channels of excitable membranes of neurons and membranes of cardiomyocytes. Reduces the duration of the action potential and the effective refractory period in the Purkinje fibers, suppresses their automatism. At the same time, lidocaine suppresses the electrical activity of depolarized, arrhythmogenic areas, but minimally affects the electrical activity of normal tissues. When used in medium therapeutic doses, it practically does not change myocardial contractility and does not slow down AV conduction. When used as an antiarrhythmic agent with intravenous administration, the onset of action is 45-90 seconds, the duration is 10-20 minutes; with i / m administration, the onset of action is after 5-15 minutes, the duration is 60-90 minutes. Route of administration and dose The dose of the drug depends on the indications and the size of the surface to be anesthetized. With each spraying of 1 portion of the spray, 4.8 mg of lidocaine (1 dose) is thrown onto the surface. In order to avoid high plasma concentrations of the drug, it is important to use the lowest dose that provides a satisfactory effect. Usually 1-2 sprays are enough, although in obstetrics 15-20 sprays or more are used (the maximum dose is 40 sprays / 70 kg of body weight). Approximate doses for various indications. In the absence of a safer method of treatment, Lidocaine-Health can also be used during pregnancy. Lidocaine is excreted into breast milk, however, at the usual therapeutic doses, the excreted amount of the drug is so small that it does not harm the child who is breastfed. Precautions Do not allow lidocaine to enter the respiratory tract (risk of aspiration). As far as possible when spraying the aerosol, the bottle should be in an upright position. Avoid getting the spray in your eyes. When taking an impression of the dentition or when placing an x-ray film, the drug can only be used when elastic impression materials are used. Before using Lidocaine-Health during operations on the pharynx and nasopharynx, it should be borne in mind that, by suppressing the gag reflex and reaching the larynx and trachea, lidocaine also inhibits the cough reflex, which can cause bronchopneumonia. This is especially important when using the drug in children, since they have a more frequent swallowing reflex. Interactions with other drugs Care must be taken when using lidocaine in patients receiving type 1B antiarrhythmic drugs (eg, tocainide), due to the risk of cumulative toxic effects. With simultaneous use with class IA antiarrhythmic drugs (including quinidine, procainamide, disopyramide), the QT interval is lengthened and, in very rare cases, AV blockade or ventricular fibrillation may develop. Phenytoin enhances the cardiodepressive effect of lidocaine. With simultaneous use with procainamide, delusions, hallucinations are possible. Lidocaine may enhance the effect of drugs that cause blockade of neuromuscular transmission, tk. the latter reduce the conduction of nerve impulses. Against the background of intoxication with digitalis glycosides, lidocaine can increase the severity of AV blockade. Ethanol enhances the inhibitory effect of lidocaine on respiration. pharmaceutical interaction. With simultaneous use, the following drugs increase the concentration of lidocaine in the blood serum: chlorpromazine, cimetidine, propranolol, peptidine, bupivacaine, quinidine, disopyramide, amitriptyline, imipramine, nortriptyline. Contraindications Hypersensitivity to the components of the drug, as well as to other amide local anesthetic drugs. If gypsum is used as an impression material when taking impressions of the dentition, Lidocaine-Health is contraindicated due to the risk of aspiration. Overdose In case of overdose, there may be adverse reactions from the central nervous system and the cardiovascular system, described in the “Adverse reactions” section. Provide fresh air, oxygen and/or artificial respiration if necessary. Convulsions resulting from overdose should be treated with succinylcholine (1 mg/kg body weight) and/or diazepam (1 mg/kg body weight IV). Since succinylcholine can cause respiratory arrest, the drug should only be used by specialists with experience in endotracheal intubation and in the treatment of patients with respiratory arrest. You can also use short-acting barbiturates, thiopental. If symptoms from the cardiovascular system (bradycardia, conduction disturbance) appear, atropine (0.5-1 mg intravenously) and sympathomimetics are prescribed. With ventricular fibrillation and cardiac arrest, immediate resuscitation is indicated. The dosage for children should correspond to the age and body weight of the patient. Side effects Local effects When using the drug, a burning sensation may occur, which disappears after the appearance of the anesthetic effect (within 1 minute). At the site of application of the drug, transient erythema, edema and decreased sensitivity may be observed. Allergic reactions Very rarely, allergic reactions can develop: urticaria, angioedema, bronchospasm, in extremely severe cases – shock. In the event of hypersensitivity reactions, the use of the drug should be discontinued. Systemic effects If Lidocaine-Health is used according to the instructions, the incidence of systemic effects is extremely low, since the amount of active substance that can reach the circulation is very small. When using high doses and in case of rapid absorption of lidocaine or with hypersensitivity, idiosyncrasy, or decreased tolerance, the following side effects may develop: reactions from the central nervous system: agitation, depression, nervousness, dizziness, drowsiness, spasms, loss of consciousness, respiratory paralysis; reactions from the cardiovascular system: arterial hypotension, myocardial infarction, bradycardia, cardiac arrest.
