Fluoroquinolones for systemic use can cause allergic reactions even after a single use. If an allergic reaction to levofloxacin occurs, the drug should be discontinued. As with other antimicrobial agents, prolonged use of the drug may lead to the growth of resistant strains of microorganisms, including fungi. If there is a worsening of the clinical picture or after a certain period of clinical improvement is not observed, it is necessary to stop using the drug and prescribe an alternative therapy. With the simultaneous use of other ophthalmic agents, the interval between instillations should be at least 15 minutes. If symptoms of a superficial bacterial eye infection are present, patients should not wear contact lenses. This medicinal product contains benzalkonium chloride and may cause eye irritation. Pharmacological properties Pharmacodynamics As an antibacterial drug of the fluoroquinolone class, levofloxacin selectively inhibits bacterial topoisomerase II-DNA-gyrase and topoisomerase IV. The preferred targets of levofloxacin in Gram-negative bacteria are DNA gyrase, and in Gram-positive bacteria, topoisomerase IV. Effective against the following microorganisms: Category I: Usually susceptible species Aerobic gram-positive: Staphylococcus aureus (MSSA – methicillin-sensitive strains), Staphylococcus pneumoniae, Staphylococcus pyogenes, Viridans group streptococci. Aerobic gram-negative: Escherichia coli, Branhamella (Moraxella) catarrhalis, Haemophilus influenzae, Pseudomonas aeraginosa (local cultures) Other organisms Chlamydia trachomatis (treatment of patients with chlamydial conjunctivitis requires concomitant systemic antimicrobial therapy) Category II: Species for which acquired resistance may be a problem Aerobic Gram-positive Staphylococcus aureus (MRSA methicillin-resistant strains) Staphylococcus epidermidis Indications Treatment of superficial bacterial eye infections caused by levofloxacin-susceptible organisms in patients 1 year of age and older. It is necessary to take into account the general recommendations for therapy with antibacterial drugs. The drug is indicated for use in adults and children aged 1 year and older. Method of application and doses For all categories of patients: for the first two days, 1-2 drops of the solution are instilled into the affected (affected) eye (s) every 2 hours, but not more than 8 times a day during the first two days, and then not more than 4 times per day for the next 3-5 days of treatment. The duration of treatment depends on the severity of the disease, the clinical course of the disease. Usually the duration of treatment is 5 days. Safety and efficacy in the treatment of corneal ulcers and neonatal ophthalmia have not been studied. Children and adolescents. Children aged one year and older use the same doses as adults. The safety and efficacy of use in children under the age of one year has not been determined. Elderly patients. There is no need for dose adjustment for elderly patients. Mode of application. For topical ophthalmic use. To prevent contamination of the drug, do not touch the dropper tip to any surface (eyelids, eye area, eyelashes, etc.). After use, the bottle must be tightly closed. However, in the case of therapeutic doses, no adverse reactions are expected in the child. Influence on the ability to drive vehicles and work with special equipmentTemporary blurred vision can affect the ability to drive a car and work with machinery and equipment. If blurred vision occurs after instillation, the patient should wait until vision is restored before driving or operating any equipment. Interaction with other drugs No special studies on the interaction of levofloxacin eye drops have been conducted. Plasma concentrations of levofloxacin when applied topically are at least 1000 times lower than when taken orally, and therefore the interaction with other drugs, characteristic of systemic use, is not clinically significant. Contraindications Hypersensitivity to levofloxacin, other quinolones or any of the excipients. Composition 1 ml of the drug contains: Active substance: levofloxacin 5 mg. Excipients: sodium chloride, benzalkonium chloride, 1M hydrochloric acid solution or 1M sodium hydroxide solution, purified water. Overdose The total amount of levofloxacin contained in one vial is too small to cause toxic reactions even after accidental ingestion. Side effects Side effects are usually mild to moderate, transient and usually limited to ocular symptoms.
