The main active ingredient Dexpanthenol + chlorhexidine Release form Cream Dosage 30 g Pharmacological properties Pharmacodynamics Dexpanthenol is the active component of Bepanthen Plus, it quickly turns into pantothenic acid in skin cells and acts as a vitamin. When applied topically, Dexpanthenol is absorbed faster than pantothenic acid. Pantothenic acid is a component of the vital coenzyme A, in the form of acetylcoenzyme A plays a central role in the metabolism of every cell. Pantothenic acid is necessary for the formation of the epithelium and the healing of damage to the skin and mucous membranes. Chlorhexidine dihydrochloride is an antiseptic, has a bactericidal effect against gram-positive bacteria, especially sensitive strains of Staphylococcus aureus, which are the most common causative agents of skin infections. Chlorhexidine dihydrochloride is less active against Gram-negative pathogens. Some species of Pseudomonas and Proteus are resistant to chlorhexidine. It has weak antifungal and no antiviral activity. Pharmacokinetics Absorption Dexpanthenol is rapidly absorbed through damaged skin. In skin cells, dexpanthenol is rapidly converted to pantothenic acid and replenishes the reserve of endogenous pantothenic acid. There is no evidence of absorption of chlorhexidine through intact skin in adults. When applied to the skin of infants with a 4% washing solution of chlorhexidine gluconate, the concentration of the latter in the blood was low (1 μg / ml). Distribution Pantothenic acid binds to plasma proteins (β-globulin and albumin). In healthy adults, its concentration in whole blood is about 500-1000 mcg / ml, and in blood serum – about 100 mcg / ml, respectively. Since chlorhexidine is only slightly absorbed through the skin, little is known about its distribution in organs or tissues. With oral administration of chlorhexidine (300 mg), the maximum plasma level of 0.2 μg / ml in a healthy adult is reached after 30 minutes. Excretion Pantothenic acid is excreted unchanged. After oral administration, 60-70% is excreted in the urine, the remainder is excreted in the feces. Chlorhexidine is practically not absorbed through the skin. When administered orally, chlorhexidine is excreted from the body almost completely with feces. Indications for use For the treatment of superficial skin lesions such as minor wounds, abrasions, cuts, scratches, burns, bites, where there is a risk of infection. As a local antiseptic in minimally invasive surgery. Method of administration and doses The cream is applied to a previously cleaned wound surface or to inflamed skin areas one or several times a day, depending on the need. Bandages may be used if necessary. Use during pregnancy and lactation There are no data indicating the need to limit the use of the cream during pregnancy and lactation. Precautions Do not use the drug on large areas of the skin, the drug is intended for small skin lesions. Avoid getting the cream in the ear canal, eyes or mucous membranes. Bepanthen Plus is not recommended for the treatment of skin irritations with a low likelihood of infection (eg sunburn). Interaction with other drugs Chlorhexidine is incompatible with saponifying agents and other anionic compounds. Bepanten Plus is not recommended to be used simultaneously with other antiseptics, to prevent their mutual influence (antagonism of action or inactivation). Contraindications Hypersensitivity to dexpanthenol and / or chlorhexidine, or to any of the components of the drug. Do not apply to a perforated eardrum. Ingredients: dexpanthenol 50 mg/g, chlorhexidine dihydrochloride 5 mg/g. Excipients: DL-pantolactone, cetyl alcohol, stearyl alcohol, white soft paraffin, liquid paraffin, lanolin (E913), macrogol stearate, purified water. Overdose Bepanthen Plus is well tolerated even at high doses, there is no risk of overdose. Even in case of accidental ingestion, no special measures are required. In case of accidental contact with the eye, slight irritation is possible, which is eliminated by rinsing with water or saline. Frequent repeated use of the drug on the same affected area can lead to skin irritation. Side effects The listed undesirable effects are based on spontaneous reports, it is not possible to determine the exact frequency of occurrence of each of them. From the side of the immune system, diseases of the skin and subcutaneous tissue: – allergic reactions, including allergic skin reactions: contact dermatitis, allergic dermatitis, itching, erythema, eczema, rash, urticaria, skin irritation, blistering rash; – hypersensitivity, anaphylactic reactions, anaphylactic shock with relevant laboratory and clinical manifestations, including symptoms of asthma, mild to moderate lesions of the skin, respiratory tract, gastrointestinal tract, cardiovascular system, including heart and respiratory failure.
