Dosage and administration The drug is used externally, both by open method and with the use of occlusive dressings. After cleansing and surgical treatment, the preparation is applied to the wound under sterile conditions with a thickness of 2-3 mm 2-3 times a day. The wound during treatment should be completely covered with cream. If part of the wound opens, additional cream must be applied. The imposition of an occlusive dressing is possible, but not mandatory. The cream is applied until the wound is completely healed or until the wound surface is prepared for skin grafting. If the drug is applied to infected wounds, exudate may appear. Before applying the cream, it is necessary to wash the wound with 0.1% aqueous solution of chlorhexidine, 3% aqueous solution of boric acid or other antiseptic. Children and adolescents: The drug should not be used in the treatment of premature, newborn and infants under 2 months of age due to the risk of kernicterus. In older children, dose adjustment is not required. Patients with renal insufficiency: Due to the lack of clinical trials in patients with renal insufficiency, caution should be exercised during the use of the drug. Serum sulfonamide concentrations should be monitored in patients with severe renal impairment. Patients with hepatic insufficiency: Due to the lack of clinical trials in patients with hepatic insufficiency, caution should be exercised during the use of the drug. Use during pregnancy and lactation Pregnant women in the prenatal and labor period should not use the drug because of the risk of kernicterus in newborns. Do not use the drug during pregnancy, except in situations where the benefit to the mother outweighs the potential risk to the fetus. Lactation It is not known to what extent topical silver sulfathiazole or its metabolites can pass into the milk of breastfeeding women. Do not use the product during breastfeeding. Precautions Possible cross-hypersensitivity to derivatives of sulfonylurea, benzothiazine and para-aminobenzoic acid. In each case, a careful allergic history should be taken, especially in relation to sulfonamides. Avoid contact of the drug with eyes and mucous membranes. In case of accidental contact, wash them with plenty of water. The drug should be used with caution in patients who are in a state of shock after extensive burns or in patients after burns with whom it is difficult to maintain contact. In patients with impaired liver and kidney function, drug accumulation is possible and there is a risk of developing systemic adverse reactions. Against the background of long-term use on large areas of the skin, the level of sulfathiazole in the blood serum should be monitored, especially in cases of kidney and liver diseases. It is also necessary to monitor the function of the liver, kidneys and blood picture, since agranulocytosis or anemia may develop. Long-term use should be avoided (more than 60 days, provided that the maximum daily dose of cream of 25 g and the total course dose of 1500 g is not exceeded). Due to the content of cetostearyl alcohol, the medicinal product may cause local reactions such as contact dermatitis. Due to the presence of propyl parahydroxybenzoate and methyl parahydroxybenzoate in the composition, allergic reactions may develop after the use of the drug. Interaction with other drugs Folic acid or derivatives of para-aminobenzoic acid (eg, procaine) may weaken the antibacterial effect of silver sulfathiazole. The occurrence of the above interactions is possible, but unlikely, since the silver salts of sulfonamides act predominantly on the cell membrane, and not on metabolic processes (but such an effect cannot be ruled out). The drug is not recommended to be used simultaneously with other topical drugs to prevent the possibility of their interaction. Contraindications Allergy to sulfathiazole, other sulfonamides or any of the components of the drug. Do not use Argosulfan: breast-feeding women; in the treatment of premature, newborn and infants up to 2 months due to the risk of nuclear jaundice; patients with congenital deficiency of glucose-6-phosphate dehydrogenase due to the risk of hemolytic anemia. Composition Active substance: 1 g of cream contains 20 mg of silver salt sulfathiazole (Sulfathiazolum argentum). Excipients: cetostearyl alcohol, white petrolatum, glycerin, liquid paraffin, sodium phosphate, sodium lauryl sulfate, potassium diphosphate, methyl parahydroxybenzoate, propyl parahydroxybenzoate, water for injection. Overdose No cases of overdose have been reported. Absorption of silver sulfathiazole through damaged skin is possible. Side effects The low solubility and poor absorption of silver sulfathiazole through damaged skin reduces the likelihood of side effects during the use of the drug. At the site of application of the cream, signs of skin irritation and hypersensitivity, such as burning, itching, erythema, may appear. In the case of prolonged use of the drug, there is a risk of developing systemic side effects characteristic of sulfonamides, including damage to the kidneys, liver, hematopoietic disorders, including agranulocytosis, hemorrhagic diathesis, thrombocytopenia, aplastic and hemolytic anemia, leukopenia.
