The main active ingredient Algeldrate + magnesium hydroxide Dosage 170 ml Indications for use Antacid therapy for heartburn and gastroesophageal reflux in patients over 15 years of age. Dosage and administration Before use, the vial with the drug should be shaken thoroughly to obtain a homogeneous suspension! The drug is not intended for use in children under 15 years of age. Take 5-10 ml 3-4 times a day 20-60 minutes after meals and at bedtime. If necessary, a single dose can be increased to 15 ml. When taking the suspension, it is not recommended to drink it with liquid. If you miss a dose, you just need to take the next one at the usual time for taking it, you should not double the dose to make up for the missed one. Do not exceed recommended doses. The duration of treatment without consulting a doctor should not exceed 10 days. The course of treatment is determined by the doctor. Use in patients with renal insufficiency. However, the possibility of monitoring pregnancies during which the use of drugs with a similar composition has taken place is too limited to exclude the possibility of a risk. Therefore, for safety reasons, the use of this medicinal product during pregnancy is possible only if clearly necessary, after a careful assessment by the physician of the potential benefit to the mother and the risk to the fetus. Long-term use of the drug or high doses should be avoided. Breastfeeding Since the absorption of aluminum and magnesium hydroxide is limited when the mother takes the drug at recommended doses, it is considered possible to continue breastfeeding during treatment. During lactation, Alugel should be used with caution, after consulting a doctor. Influence on the ability to drive vehicles and maintain mechanical equipment The presence of ethyl alcohol in the composition of the drug should be taken into account, despite the fact that ethanol is contained in quantities that do not affect the ability to drive vehicles or work with mechanisms when taking the recommended daily doses of the drug. Precautions It is necessary to observe a 2-hour interval between the use of Alugel and other drugs (when taking antimicrobial agents of the fluoroquinolone series – 4 hours). The patient should contact the attending physician in case of: – weight loss; – difficulty swallowing and persistent abdominal discomfort; – digestive disorders that appeared for the first time, changes in existing digestive disorders; – renal failure. Aluminum salts are generally poorly absorbed from the gastrointestinal tract and therefore systemic effects are rare in patients with normal renal function. However, taking excessive doses of the drug or its long-term use, or even the use of normal doses in patients whose diet is characterized by a low content of phosphorus, can lead to a decrease in the content of phosphate in the body, accompanied by an increase in bone resorption processes and the occurrence of hypercalciuria with an increased risk of osteomalacia. (due to the binding of aluminum to phosphate). In this regard, you should regularly consult a doctor in case of prolonged use or if there is a risk of phosphorus deficiency in the body. The duration of treatment without consulting a doctor should not exceed 10 days. If during treatment the symptoms persist for more than 10 days or the condition worsens, you should definitely consult a doctor so that he can establish the cause, double-check the correctness of the diagnosis and review the treatment. In patients with renal insufficiency, elevated plasma concentrations of both aluminum and magnesium are observed. In such patients, long-term use of high doses of aluminum and magnesium salts can lead to the development of encephalopathy, dementia, microcytic anemia, or worsen the course of dialysis-induced osteomalacia. Aluminum hydroxide can be dangerous for porphyria patients on hemodialysis. Not recommended for use in patients with severe chronic constipation, suspected acute appendicitis, metabolic alkalosis, liver cirrhosis, severe heart failure, toxemia of pregnancy, ulcerative colitis, diverticulosis, colostomy or ileostomy (increased risk of developing fluid and electrolyte imbalance), with chronic diarrhea, exacerbation of hemorrhoids, impaired renal function (creatinine clearance <30 ml / min, risk of hypermagnesemia and aluminum intoxication). The use of antacids containing aluminum in elderly patients should be limited. With prolonged use in such patients, the condition of existing diseases of the bones and joints may worsen. The drug Alugel contains sorbitol, which allows its use in patients with diabetes mellitus, but should not be taken in patients with congenital fructose intolerance, as this can cause stomach irritation and diarrhea. The calorie content of one dose of Alugel (5 ml of suspension) is 1.5 kcal; 10 ml of suspension - 3.0 kcal; 15 ml of suspension - 4.5 kcal; the maximum daily dose (40 ml) - 12 kcal. Alugel contains parabens (parahydroxybenzoates), as a result of which the use of this drug may cause allergic reactions (possibly delayed). The content of ethanol (ethyl alcohol) in the medicinal product is 2.5 vol. %, i.e. 98.1 mg in a single dose (5 ml), which is equivalent to 2.5 ml of beer, 1 ml of wine or 196.2 mg of ethanol in a dose of 10 ml of the drug, which is equivalent to 5 ml of beer, 2 ml of wine, or 294.3 mg of ethanol at a dose of 15 ml, which is equivalent to 7.5 ml of beer, 3 ml of wine. It is dangerous to apply to patients suffering from alcoholism. It should be taken into account in pregnant women, nursing mothers, children, as well as patients at high risk, including those with liver disease and epilepsy. Interactions with other drugs Antacids interact with some other oral medications. Contraindications - Hypersensitivity to active substances or any of the auxiliary components of the drug. - Rare congenital fructose intolerance. - Severe renal insufficiency (since the medicinal product contains magnesium). - Age up to 15 years. - Porfiria. - Cachexia. Each 5 ml suspension contains active substances: aluminum hydroxide gel in terms of aluminum oxide - 218 mg and magnesium hydroxide paste in terms of magnesium oxide - 75 mg; excipients: non-crystallizing sorbitol solution, ethyl alcohol 96%, hydroxyethyl cellulose, methyl parahydroxybenzoate (E218), propyl parahydroxybenzoate (E216), butyl parahydroxybenzoate, lemon oil, sodium saccharin, purified water. Overdose Magnesium overdose does not usually cause toxic reactions in patients with normal renal function. However, magnesium poisoning can develop in patients with renal insufficiency (see section "Precautions"). Symptoms of an acute overdose of aluminum hydroxide and magnesium hydroxide combination include diarrhea, abdominal pain, and vomiting. High doses of the drug may cause or exacerbate intestinal obstruction in patients with an increased risk of such complications. The toxic effect depends on the concentration of magnesium in the blood serum. Signs of toxic action: - decrease in blood pressure; - nausea, vomiting; - drowsiness, decreased reflexes, muscle fatigue, neuromuscular paralysis; - bradycardia, deviation of ECG results; - hypoventilation; - in the most severe cases, respiratory paralysis, coma, impaired renal or cardiac function may develop; - anuric syndrome. Aluminum and magnesium are excreted through the urinary tract. Treatment of magnesium overdose: rehydration, forced diuresis. In case of renal insufficiency, hemodialysis or peritoneal dialysis is necessary. Interaction with other medicinal products and other types of interactions Antacids interact with some other drugs taken by mouth. Adverse reactions Adverse reactions are classified according to frequency and system organ class. The risk is increased with concomitant oral administration of citric acid, sodium citrate, or calcium citrate. Hypophosphatemia may develop after long-term use or high doses, as well as use at recommended doses in patients whose diet is characterized by a low content of phosphorus, which can lead to increased bone resorption processes and the occurrence of hypercalciuria with an increased risk of osteomalacia.
