Heparin is a direct-acting anticoagulant that belongs to the group of medium molecular weight heparins. When applied externally, it has a local anticoagulant, moderate anti-inflammatory effect. Benzocaine has a local anesthetic effect. Pharmacokinetics When applied topically, heparin is slightly absorbed. Communication with plasma proteins – up to 95%. It does not cross the placenta and into breast milk. It is metabolized in the liver with the participation of N-desulfamidase and platelet heparinase. Desulfated molecules under the influence of kidney endoglycosidase are converted into low molecular weight fragments. The half-life is on average 1.5 hours. It is excreted by the kidneys, mainly in the form of inactive metabolites. Benzocaine is practically not absorbed when applied topically. Indications for use Thrombosis of external and internal hemorrhoids. Dosing and Administration Rectally. The suppository is introduced into the rectum after a spontaneous bowel movement or cleansing enema. Apply 1 suppository 2 times a day. The course of treatment is 10-14 days. When using rectal suppositories, abdominal cramps and an urge to defecate often occur, so before the procedure, perform a cleansing enema or empty the intestines naturally. Wash your hands thoroughly with soap. Prepare wet wipes or a handkerchief soaked in water in advance. Lie on your side with your knees pressed to your stomach. For the procedure, this position is the most convenient, as it allows you to minimize discomfort. Remove the candle from the package. The candle is taken with the thumb and middle fingers of the right hand in the middle of the widest part, the index finger rests on the blunt end of the candle. The candle is directed with the free end to the anus and pushed with the index finger into the anal canal until it is completely immersed to a depth of 2-3 cm. Carry out the procedure carefully, without exerting excessive pressure. Despite the fact that the suppository has a streamlined shape, careless use can injure the mucous membrane. Do not get out of bed for about 20 minutes. Wipe your hands with a pre-prepared damp cloth or handkerchief. If more than 10 minutes have passed between the introduction of the suppository and a bowel movement, an additional dose of the drug is not required. Use during pregnancy and lactation The safety of Nigepan® during pregnancy and lactation has not been established. Precautions When used in combination with other anticoagulant agents, careful monitoring of blood coagulation parameters is necessary. If the existing symptoms of the disease do not decrease during treatment, if anal bleeding occurs, blood is suspected in the stool, and if new unusual symptoms appear, you should consult a doctor. Benzocaine is metabolized to para-aminobenzoic acid, which may interfere with the effects of sulfonamides. Contraindications Hypersensitivity to the components of the drug, bleeding disorders, thrombocytopenia, hypocoagulation; heparin-induced thrombocytopenia at present or in history; childhood. One suppository contains: active ingredients: sodium heparin – 1000 IU (8.3 mg with sodium heparin activity 120 IU / mg), benzocaine – 50 mg; excipients: water – 19.5 mg; solid fat (Witepsol, Supposir) – to obtain a suppository weighing 1.25 g, distilled monoglycerides (Palsgaard 0093) – up to 5% to the amount of solid fat. Overdose There is no information on cases of overdose.
